Job Summary
We are seeking warm and razor-smart Psychiatric Nurse Practitioners with entrepreneurial spirits to provide psychiatric consultations in nursing homes. This is a travel-based role that combines in-person visits at assigned nursing facilities with telehealth services. Candidates must be comfortable traveling throughout Vermont to provide in-person patient care while also conducting consultations and follow-ups via telehealth.
As a Psychiatric Nurse Practitioner, you will be provided with full administrative support and clinical mentorship. Responsibilities
- Travel to assigned nursing homes throughout Vermont to provide in-person psychiatric consultations and patient care.
- Conduct psychiatric consultations and follow-up appointments through both in-person visits and telehealth.
- Work closely and collaboratively with nursing home staff, residents, families, and other members of the care team.
- Proactively follow up with patients and assess the efficacy of recommended interventions.
- Provide education to patients, families, and facility staff on psychiatric best practices.
- Round at assigned nursing homes and provide psychiatric medication and treatment recommendations.
- Integrate pharmacological and behavioral interventions, emphasizing non-pharmacological treatments whenever possible.
- Maintain timely, accurate, and thorough clinical documentation.
Requirements
- Master of Science in Nursing (MSN)
- Active Psychiatric Nurse Practitioner License
- ANNC Board Certification
- Ability and willingness to travel throughout Minnesota for in-person nursing home visits
- Comfortable providing care through both in-person and telehealth modalities
- Excellent communication and documentation abilities
- Strong patient assessment skills
- Strong, ambitious, patient-centric work ethic
- Prolonged sitting. Computer work including keyboard and mouse use for extended periods. Lifting/moving patients.
