NorthPointe Clinic is seeking a compassionate, skilled, and patient-focused Certified Hand Therapist (CHT) - Occupational Therapist to join our dedicated rehabilitation team. In this role, you will provide specialized evaluation and treatment services for patients with upper extremity conditions, including injuries and disorders of the hand, wrist, elbow, and shoulder. The ideal candidate is committed to delivering exceptional patient care and collaborating with physicians, therapists, and interdisciplinary team members to achieve optimal patient outcomes.
- Evaluate, develop, and implement individualized treatment plans for patients with upper extremity injuries and conditions.
- Provide evidence-based therapeutic interventions to improve function, mobility, strength, and independence.
- Fabricate, fit, and modify custom orthoses/splints as clinically indicated.
- Educate patients and families regarding treatment plans, home exercise programs, injury prevention, and self-management strategies.
- Accurately document evaluations, treatment interventions, progress notes, and discharge summaries in accordance with regulatory and organizational standards.
- Collaborate with orthopedic surgeons, hand specialists, primary care providers, and rehabilitation team members to coordinate patient care.
- Monitor patient progress and adjust treatment plans to maximize outcomes.
- Maintain current knowledge of hand therapy best practices, clinical techniques, and regulatory requirements.
- Participate in quality improvement initiatives and departmental activities
JOB REQUIREMENTS:
- Bachelor or Master's in Occupational Therapy or Certificate degree in Occupational Therapy from an accredited school,
- Expertise in upper extremity rehabilitation and hand therapy techniques.
- Strong patient education and motivational skills.
- Ability to work independently while collaborating effectively within a multidisciplinary team.
- Excellent organizational, documentation, and time management skills.
- Commitment to providing compassionate, patient-centered care.
- Current Occupational Therapy License in Illinois
- CPR Certification within 3 months of hire
