The Medication Assisted Treatment (MAT) Coordinator is responsible for coordinating the Jail Medication Assisted Treatment (JMAT) program and providing substance use disorder services to incarcerated individuals diagnosed with opioid use disorder. Responsibilities include conducting substance use assessments, providing individual and group counseling, educating patients on medication-assisted treatment, coordinating referrals to internal treatment services, maintaining clinical documentation, monitoring program outcomes, and collaborating with medical, behavioral health, detention, and community partners to support evidence-based treatment within the correctional setting. The MAT Coordinator assists with Medicaid eligibility verification and enrollment for program participants and collaborates with the facility's Re-Entry Coordinator to facilitate continuity of care upon release. The position also participates in quality improvement initiatives, program evaluation, and data collection to support the effectiveness and ongoing development of the JMAT program.

1. Collaborate with nursing staff, providers, and behavioral health personnel to identify inmates who report opioid use disorder (OUD) or other substance use disorders during the comprehensive medical intake screening. 
2. Review referrals generated from the Clinical Opiate Withdrawal Scale (COWS) assessments and meet with inmates who qualify for Jail Medication Assisted Treatment (JMAT). 
3. Complete substance use assessments, including the TCU-5 and other validated screening tools as appropriate, to determine treatment needs and level of care. 
4. Educate inmates regarding opioid use disorder, medication-assisted treatment (MAT), recovery principles, overdose prevention, relapse prevention, and program expectations. 
5. Obtain informed consent for participation in the Jail Medication Assisted Treatment (JMAT) program and document all required program enrollment activities. 
6. Provide individual counseling, case management, and psychoeducational groups utilizing evidence-based practices consistent with correctional treatment standards. 
7. Document all patient encounters, counseling sessions, assessments, treatment plans, group participation, referrals, and discharge planning activities in the electronic health record. 
8. Coordinate referrals to internal jail programs including, but not limited to: 
   • Jail Medication Assisted Treatment (JMAT) 
   • Comprehensive Opiate Abuse Program (COAP) 
   • Jail Behavioral Health Services 
   • Behavioral Health Assistance Program (BHAP) 
   • Jail Education and Transition (JET) 
9. Coordinate referrals to psychiatric services and collaborate with behavioral health providers regarding co-occurring mental health disorders. 
10. Participate in multidisciplinary treatment team meetings with medical, psychiatric, nursing, behavioral health, and custody staff regarding patient progress and continuity of care. 
11. Verify Medicaid eligibility and other insurance coverage for inmates participating in MAT services. 
12. Coordinate Medicaid enrollment or reactivation with Boulder County Human Services to minimize interruptions in medication-assisted treatment following release. 
13. Coordinate prior authorization requirements, medication continuity, and follow-up appointments with community opioid treatment providers, primary care providers, behavioral health providers, and pharmacies prior to inmate release. 
14. Communicate and coordinate continuity of care with community partners including probation, parole, public health agencies, community mental health centers, opioid treatment programs (OTPs), Federally Qualified Health Centers (FQHCs), recovery organizations, and other treatment providers. 
15. Provide naloxone (Narcan) education and overdose prevention counseling. Coordinate naloxone distribution when available and appropriate. 
16. Maintain current knowledge of evidence-based treatment for opioid use disorder, Colorado Medicaid requirements, and community treatment resources. 
17. Collect, analyze, and maintain program performance data including participation rates, treatment completion, linkage to community treatment, recidivism, overdose prevention outcomes, and other quality improvement measures. 
18. Prepare monthly, quarterly, annual, and grant-required statistical reports. 
19. Participate in quality improvement initiatives and assist with accreditation activities, audits, and program evaluations. 
20. Attend professional meetings, regional committees, and statewide collaborative groups related to opioid use disorder treatment, including SUAG, SUAG Steering Committee, the Colorado Consortium, and other applicable organizations. 
21. Perform other duties as assigned. 

KNOWLEDGE, SKILLS, AND ABILITIES:

• Knowledge of opioid use disorder, substance use disorders, medication-assisted treatment, motivational interviewing, relapse prevention, and recovery-oriented systems of care. 
• Knowledge of correctional health care practices and security procedures within a detention facility. 
• Ability to conduct comprehensive substance use assessments and develop individualized treatment plans. 
• Ability to establish therapeutic relationships while maintaining appropriate professional boundaries. 
• Strong organizational, case management, and care coordination skills. 
• Ability to communicate effectively with inmates, medical staff, behavioral health professionals, custody staff, courts, community agencies, and outside treatment providers. 
• Ability to maintain accurate clinical documentation within an electronic health record. 
• Knowledge of Medicaid eligibility, insurance coordination, and community treatment resources is preferred. 
• Ability to collect, interpret, and analyze program outcome data. 
• Ability to maintain confidentiality and comply with HIPAA 
• Strong written and verbal communication skills. 

EDUCATION AND EXPERIENCE:

Required Licensure (one of the following)

• Licensed Addictions Counselor (LAC) 
• Licensed Professional Counselor (LPC) 
• Licensed Clinical Social Worker (LCSW)

Preferred

• Experience working in a correctional, detention, residential treatment, or behavioral health setting. 
• Experience providing medication-assisted treatment (MAT) services. 
• Experience with Medicaid enrollment, care coordination, or discharge planning. 
• Experience documenting behavioral health services in an electronic health record and maintaining documentation that supports Medicaid reimbursement is preferred.
• Bilingual (English/Spanish) skills are desirable.

SECURITY REQUIREMENTS:

FUNCTIONAL REQUIREMENTS:

1. Vision - Dual vision is not required. Single eye correctable vision is acceptable.  Minimum peripheral vision for single vision is 90⁰ on the ear side and 70⁰ on the nose side.  Frequently uses peripheral vision to observe and monitor multiple people and activity.

2. Hearing - Dual hearing is required.  When tested by use of an audiometric device, does not have an average hearing loss in the better ear greater than 40 decibels at 500 Hz, 1000 Hz, and 2000 Hz with or without a hearing aid when the audiometric device is calibrated to American National Standard.  Staff are constantly required to listen to multiple audible inputs simultaneously such as portable radio conversations, telephone conversations, conversations with inmates or others, noises within the housing units.

3. Smell - Intact.  Occasionally uses olfactory sense to detect illegal use of drugs or chemicals.   Occasionally uses the olfactory sense to evaluate the presence of toxic vapors, smoke, and fuels.

4. Speech – Staff must have the ability to speak and understand English clearly within normal parameters. Constantly uses voice to communicate with other officers, professionals, prisoners, and the public in person and by telephone and two-way radio.  Clear diction and audible volume is required.

5. Body Movement – Staff must have mobility of both arms and legs with the ability to move around the work area unassisted.  Must be able to walk quickly and climb stairs.  Must have the ability and range of flexibility to reach overhead, reach below knees, and to bend over or squat down to perform job functions.   Must have the ability to stand or sit for long periods of time.
