Description
Some nurses thrive in the controlled chaos of a hospital floor. Others would rather be the calm, tablet-wielding hero who shows up at a patient's front door and makes the whole care plan make sense. If you are the second kind, keep reading.
As a Registered Nurse Case Manager on our home health team, you are the quarterback of patient care. You coordinate everything from the first assessment to the final discharge, and you do it in living rooms, kitchens, and the occasional front porch across the greater Omaha area. You are part clinician, part detective, part diplomat, and entirely dedicated to helping people heal where they are most comfortable.
What You Will Do
- Conduct comprehensive in-home assessments and develop individualized plans of care that actually fit the patient's life, not just the textbook.
- Serve as the primary case manager for an assigned caseload, coordinating with physicians, therapists, aides, and family members so everyone is reading from the same chart.
- Perform skilled nursing visits, including wound care, medication management, IV therapy, and patient and caregiver education.
- Complete accurate and timely OASIS documentation and keep clinical records compliant with Medicare and state regulations.
- Monitor patient progress, adjust care plans as conditions change, and recognize when to escalate.
- Supervise and guide LPNs and home health aides contributing to each patient's care.
- Advocate for patients and their families, connecting them with community resources and making the complicated feel manageable.
What We Are Looking For
We need a nurse who can read a care plan and a room with equal skill. The ideal candidate brings clinical confidence, genuine warmth, and the kind of organizational superpowers that keep a full caseload from turning into a tangle.
- Current, unencumbered license as a Registered Nurse in the state of Nebraska.
- At least one year of nursing experience; home health or case management experience is a strong plus.
- Working knowledge of OASIS documentation, Medicare guidelines, and home health regulations preferred.
- Comfort with electronic medical records and the patience to document thoroughly.
- A valid driver's license, reliable transportation, and a willingness to travel to patient homes throughout the Omaha region.
- Excellent communication skills and the ability to work independently while staying connected to the wider care team.
- Strong clinical judgment, critical thinking, and a steady bedside manner.
Why This Role Matters
Home health is where medicine meets real life. Your assessments and decisions directly shape whether a patient recovers safely at home or ends up back in the hospital. You will carry real autonomy and real responsibility, and you will see the impact of your work in faces, not just flowsheets.
