Description
- Provide personal care to hospice patients, including bathing, grooming, oral care, and dressing.
- Assist with mobility, transfers, ambulation, and positioning for comfort.
- Support patients with light housekeeping, and maintaining a safe home environment.
- Report changes in patient condition, behavior, or environment to the assigned Case Manager promptly.
- Follow and implement the patient’s plan of care as directed by the RN.
- Maintain accurate, timely, and complete visit documentation in accordance with agency policy and hospice regulations.
- Adhere to all infection control, safety practices, HIPAA, and agency protocols.
- Participate in team meetings and in-service training as required.
- Active Home Health Aide (HHA) Certificate in the state of California (required).
- Hospice or home health experience preferred, but not required—training provided.
- Current CPR Certification.
- Valid California Driver’s License, reliable vehicle, and proof of auto insurance.
- Ability to work independently in the field and follow care plans accurately.
- Strong communication skills and a compassionate, patient-centered approach.
