We’re looking for a Hospice LPN to join our team and make a meaningful difference in the lives of patients and families. As part of an RN/LPN care team, you’ll help deliver compassionate, high-quality hospice care to patients living in nursing homes, long-term care facilities, and assisted living communities, primarily throughout Fauquier and Prince William Counties. Enjoy a consistent Monday-Friday, 8:30 a.m.-5:00 p.m. schedule while doing work that truly matters.
Responsibilities:
· Provide compassionate, skilled nursing care focused on comfort, dignity, and quality of life.
· Monitor patients, recognize changes in condition, and communicate promptly with the RN Case Manager and care team.
· Administer medications and treatments according to each patient’s individualized plan of care.
· Educate and support patients and families through symptom changes, comfort measures, and the end-of-life journey.
· Collaborate with RNs, physicians, hospice aides, social workers, and other team members to provide seamless, coordinated care.
· Provide a reassuring presence to patients and families, including compassionate support at the time of death.
· Maintain accurate clinical documentation while upholding patient privacy, infection prevention, and standards of care.
· Participate in team collaboration, quality improvement, and ongoing professional development.
Qualifications:
· Licensed as a Licensed Practical Nurse in the Commonwealth of Virginia
· Two years’ relevant nursing experience preferred
· Driver’s license, personal automobile, and current automobile liability insurance

· Drive long distances to homes and facilities
· Must be prepared to work in home environments that have difficult working conditions including, but not limited to cramped space, pets, dirt, allergens smoke, limited seating
· The job requires regular use of hands for handling objects and involves bending, reaching, walking, and climbing steps. Vision requirements include color, peripheral, and the ability to adjust focus
· It may be necessary at times for employees to work beyond scheduled work hours to ensure adequate patient care
· Communication by phone, writing, speaking; Regular use of a computer screen and keyboarding
· Able to push, pull, or lift 50 pounds, push up to 200 pounds in a wheelchair
