Summary

The Internal Medicine Nurse Practitioner (NP) provides advanced nursing care to adult patients with acute, chronic, and complex medical conditions. Working in collaboration with physicians and healthcare teams, the NP performs assessments, diagnoses, develops and implements treatment plans, and manages patient care to promote optimal health outcomes.

Job Description

Conduct thorough patient histories and physical examinations. Diagnose and manage acute and chronic illnesses such as hypertension, diabetes, COPD, heart disease, and other internal medicine conditions. Order, interpret, and act on diagnostic tests including labs, imaging, and screenings. Develop and implement individualized treatment plans, including prescribing medications and therapies in accordance with state laws and organizational policies. Provide patient education on disease prevention, health promotion, and management of chronic conditions. Monitor and evaluate patient progress and response to treatment, adjusting plans as necessary. Collaborate with physicians, specialists, and other healthcare providers to coordinate care. Document all patient encounters, treatments, and progress notes accurately in the electronic health record (EHR). Participate in quality improvement initiatives and stay current with evidence-based practices in internal medicine. Maintain compliance with regulatory and organizational standards for patient care.

Qualifications

Master’s or Doctorate degree from an accredited Nurse Practitioner program.

If APRN was licensed prior to August 1, 1995, master's degree in nursing is preferred.


Experience in internal medicine or related adult primary care setting preferred

Current licensure as a Registered Nurse and Nurse Practitioner in each state of practice.

·    Schedule : M-F; closed 6 major holidays. Walk-in-Clinic Saturday rotation 9 am-noon; 1:6

·       Clinic only         

·       New grads with 3-5 years of nursing experience in primary care will be considered; Experience as an APP in primary care/internal medicine of greater than 1 year preferred.

·       Position duties: Asses/Evaluate and develop appropriate history, physical, assessment and plan for complex internal medicine primary care patients.  Position would include own primary panel of patients as well as working collaboratively to share patients with internal medicine physicians.

·       Make up of Team: Physicians: 2, APP’s: 1

 

Additional primary care providers in family medicine consist of 3 physicians and 3 APPs as well as 2 OBGYN/Women’s Health physicians and 2 APPS.

 

·       Required 1.0 FTE; 36 hours per week of patient facing scheduled time

Current Drug Enforcement Administration (DEA) permit to prescribe controlled substances in each state of practice.

State Controlled Substance License (if required by State)

National board certification as a Family (FNP-BC) or (FNP–C), Adult-Gerontology Primary Care (AGPCNP-BC) or (A-GNP), Adult (ANP-C), Additional certifications require approval from a market executive leader: Adult-Gerontology Acute Care (AGACNP-BC) or ACNPC-AG), Acute Care Adult (ACNP-BC)

Obtains and subsequently maintains required specific competencies and certifications as outlined on privilege form.
