Transplant Coordinator RN

Job Summary and Responsibilities

As our Transplant Coordinator RN, you will coordinate all aspects of renal and pancreas transplants and support daily functioning of the transplant section.

 

Every day you will act as case manager using Virginia Mason's Nursing Professional Practice Model, managing patients from initial referral through transplant evaluation, the inpatient stay after transplant, and the transition back to the referring physician.

 

To be successful in this role, you will bring RN transplant experience and strong case management skills.

 

• Comprehensive Case Management: Manage the entire transplant journey using the Nursing Professional Practice Model, overseeing patients from initial referral and evaluation through the inpatient stay and long-term post-transplant transitions.
• Clinical Coordination: Facilitate all aspects of renal and pancreas transplants by scheduling diagnostic testing, collaborating with multidisciplinary teams, and coordinating care between inpatient and outpatient settings.
• Recipient & Donor Education: Provide expert education and support to transplant recipients, families, and living donors across multiple regions, ensuring they are informed and prepared for the donation and recovery process.
• Living Donor Management: Oversee all pre- and post-donation activities, including medical testing coordination, family communication, and clinical support in the operating room.
• Clinical Oversight & Triage: Conduct inpatient rounds, perform post-operative clinic assessments, and triage patient concerns to ensure long-term health and safety post-transplant.
• Outreach & Collaboration: Act as a key liaison between patients, referring physicians, and dialysis units, while facilitating multidisciplinary support groups and educational programs.
