Your Role as Health Liaison Officer – Mental Health Multidisciplinary Team

The Health Liaison Officer plays a key role in connecting Wesley Mission Queensland’s Mental Health Multidisciplinary Team with general practice and primary care providers. Working within a multidisciplinary team, this role builds strong stakeholder relationships, supports referral pathways and shared care arrangements, and helps ensure coordinated, person-centred care across the community. Based at a WMQ site, the role actively engages with stakeholders in community and primary care settings.

Key responsibilities of this role include:

  • Building and maintaining effective referral pathways and shared care arrangements with general practitioners, practice managers and primary care stakeholders.

  • Developing and sustaining strong relationships with general practices, primary care networks and community partners to support service access and integration.

  • Promoting the Mental Health Multidisciplinary Team program and act as a key liaison between WMQ and external stakeholders.

  • Supporting coordinated, person-centred care by facilitating communication and collaboration across multidisciplinary and primary care teams.

  • Assisting with care coordination activities to ensure smooth transitions between the program and a participant’s broader care network.

  • Identifying opportunities to strengthen stakeholder engagement, service accessibility and referral outcomes.

  • Representing WMQ within the community, engaging stakeholders in their own settings to build awareness, partnerships and service connections.

What We’re Looking For

  • Relevant tertiary qualification in Public Health, Community Development, Social Work, Human Services, or a related discipline, or equivalent experience.

  • Experience building stakeholder relationships and referral pathways within primary care, healthcare, or community services.

  • Strong community engagement, partnership development, or advocacy experience.

  • Ability to deliver presentations and communicate effectively with a range of audiences.

  • Understanding of and commitment to working with diverse communities, including Aboriginal and Torres Strait Islander peoples, CALD, Pasifika and LGBTQIA+ communities.

A Valuable Addition, Yet Not a Must-Have!

  • Knowledge of existing networks within general practice or primary care in the Brisbane South region. 

  • Experience working within a multidisciplinary mental health team. 

  • Knowledge of the Initial Assessment and Referral Decision Support Tool (IAR-DST). 

  • Experience developing engagement strategies, collateral or communications for health programs.

Essential Requirements

  • Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)

  • Relevant probity checks required by legislation and WMQ Limited policy.

  • Current Driver’s Licence and willingness to travel for work
