Overview

Presbyterian Homes & Services is seeking a Provider Relations Representative to increase profitable enrollment in Optage Meals by establishing, maintaining, and servicing relationships with providers. This role focuses on outreach, relationship-building, and ongoing support to ensure providers are engaged and equipped to refer participants to the Optage Meals program.

Responsibilities

Key responsibilities include but are not limited to:

  • Identify and build partnerships with key healthcare providers, community organizations, and referral sources including managed care organizations, health plans, public health departments, hospitals and clinics, VAs, food shelves, government agencies, nursing homes, low-income housing, home health agencies, congregate sites, churches, community centers, and Area Agencies on Aging.
  • Implement strategies to establish strong provider relationships that drive enrollment growth.
  • Conduct regular outreach to providers to promote the benefits of Optage Meals and address their needs.
  • Deliver educational sessions, presentations, and materials to increase provider awareness and program participation, including collateral creation and drop-offs, social media, health fairs, conferences, expos, and site tours.
  • Serve as the primary contact for providers, offering prompt responses to inquiries and proactive resolution of issues.
  • Identify and manage service recovery issues with referring agencies and, when needed, directly with Optage Meals customers.
  • Serve as the primary responsible party for growing new counties as delivery boundaries expand.
  • Monitor referral trends and enrollment metrics to identify growth opportunities.
  • Maintain a comprehensive database of referral sources to track clients by payor source, county, organization, and case manager over time.
  • Manage specific plans for external referral sources, including enrollment goals, key decision-makers to influence, and barriers to overcome.
  • Ensure all provider-related activities comply with organizational policies, regulatory standards, and confidentiality requirements.

Qualifications

  • Bachelor's degree with concentration in Marketing, Communications, Business, Health Care Administration, or related field.
  • Demonstrated knowledge and experience in customer service, sales, and community outreach.
  •  Experience working with seniors and their families in senior housing, long term care, homecare, hospice, primary care, or health care related environment preferred.
  • Strong knowledge of Hospice, Home Care, and Primary Care services and awareness of other community services.
  • Familiarity with Medicare health insurance offerings and enrollment processes.
  • Strong interpersonal and communication skills with the ability to engage diverse stakeholders in a manner that generates confidence in Optage services.
  • Analytical and problem-solving skills to identify opportunities and drive program success.
  • Self-motivated and organized, with the ability to manage multiple relationships and tasks effectively.

Pay Range

The estimated starting pay range for this role is $24.35 - $30.44 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
