ASVH-Flagstaff – LPN/RN Wound Care Nurse
Location: 2100 N Gemini Rd, Flagstaff, AZ
Schedule: Flexible (7am–3pm, 8am–4pm, or 8am–5pm), Monday–Friday, 8 hours/day. No weekends.
Position: 1 Full-Time

We are seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide specialized wound care and support residents’ skin health.

Key Responsibilities:

• Perform wound assessments, management, and reassessments per policies and procedures.

• Develop and implement skin care programs to prevent breakdown.

• Manage residents with draining wounds, fistulae, ostomies, and incontinence.

• Revise and evaluate plans of care; assess and manage pain.

• Serve as a resource for nursing staff and provide in-services on wound care, ostomy care, and prevention strategies.

• Communicate effectively with staff, physicians, and interdisciplinary teams.

• Maintain accurate documentation and stay current on wound care advancements through training and continuing education.

Requirements:

• Current Arizona RN or LPN license.

• Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN).

• CPR/BLS certification.

• Fingerprint Clearance Card issued by the Arizona DPS.

• In-state travel as needed.

Preferred:

• Experience in a skilled nursing or long-term care facility.

Resume, Fingerprint Clearance, Background Check (7 years, OIG/SAM), CPR/BLS Card, TB Test, License/Certification, Skills Assessment, Hepatitis B vaccine or declination, Competency Tests, Drug Test, COVID Vaccination, APS.
