A leading medical institution is seeking a Patient Navigator, Administrative Assistant to support patients receiving substance use disorder treatment by coordinating appointments, conducting patient outreach, reducing barriers to care, navigating healthcare resources, and providing administrative support.
Patient Navigator, Administrative Assistant Responsibilities:
- Conduct outreach and engagement for patients referred to the clinic by scheduling appointments, coordinating follow-up care, facilitating access to services, reducing barriers to treatment, and supporting patients throughout their healthcare journey using a patient-centered, strengths-based, and trauma-informed approach.
- Serve as a non-clinical liaison for patients and families by providing guidance on healthcare resources, health insurance, primary care, behavioral health, social services, residential treatment, and other community resources while promoting patient satisfaction and positive care outcomes.
- Collaborate with substance use counselors, providers, and multidisciplinary care teams to support treatment planning, maintain accurate documentation, coordinate patient care, communicate patient needs, and ensure timely access to medications and addiction treatment services.
- Perform front-desk and administrative responsibilities, including patient scheduling, appointment management, registration support, Epic documentation, communication with patients and community partners, and general clinic operations, while maintaining accurate records and protecting patient confidentiality.
Patient Navigator, Administrative Assistant Qualifications:
- Education in healthcare, behavioral health, social services, human services, or a related field is preferred.
- Experience working with underserved communities, individuals with substance use disorders, behavioral health populations, or other vulnerable patient populations.
- Experience with patient outreach, patient navigation, case coordination, healthcare administration, front desk operations, or medical office support.
- Experience scheduling patient appointments and managing patient records using Epic Hyperspace or a similar electronic health record system.
- Knowledge of healthcare systems, community resources, insurance navigation, harm reduction principles, trauma-informed care, culturally responsive care, and patient-centered service delivery.
- Lived experience with substance use recovery and/or strong connections to the local communities served is preferred.
Shift:
- 10:30 AM to 7:00 PM.
