Your Responsibilities:
- Assist Physical Therapists in establishing and administering treatment plans with specific therapy goals.
- Help assess patient needs; plan, evaluate, and modify care to meet physical therapy intervention goals.
- Collaborate with all disciplines to plan and evaluate team goals for each patient.
- Support in setting realistic and achievable goals for patients/residents.
- Document and record patient/resident condition and progress per state and federal regulations.
- Educate patients and families in appropriate therapy methods.
- Optimize patient functionality and skills by assisting in the screening of all residents.
- Communicate patient status and needs to the patient, family, and other involved professionals.
