Summary: Each Case Manager will be responsible for working with clients and other Plains Area Mental Health Center team members to enhance treatment through client engagement. Case Managers are responsible for the engagement with clients, overseeing the agency treatment plan, and completing assessments and updates with the client directly to meet agency compliance timeframes. Case Managers will find appropriate resources to address Social Determinates of Health for clients and may assist with urgent needs such as housing, food, etc. Case Managers will manage the communication of court ordered clients, working to engage with clients and coordinate care between providers. This position will also be responsible for collaborating with various entities throughout the catchment area including, but not limited to, emergency departments, urgent care clinics, inpatient medical health units, and inpatient behavioral health units. The Case Manager will be responsible for finding appropriate resources for clients that may need assistance with urgent immediate needs such as housing, food, no electricity/gas/water, clothing, or other personal needs. Additionally, Case Managers will be responsible for assisting with immediate physical health needs, immediate inpatient needs for SUD/MH, and safety concerns that would warrant a court order to services. Case Managers will play a central role in team-based care. This is an unpaid internship that can be used for school credit.

Essential Duties and Responsibilities:
- Enhance client engagement in treatment programs through direct client interactions.
- Work with clients and other team members to creatively explore alternatives to hospitalization, higher levels of care, homelessness, and incarceration.
- Advocate for clients to help meet their needs, desires, and best interests, especially during crisis situations.
- Utilize critical thinking in making independent judgements and maintain responsibility and accountability for service provided.
- Knowledge of the effects of mental illness and the ability to relate respectfully to clients with a wide range of abilities and diagnoses.
- Possess teamwork skills including problem solving, cooperation, negotiating, etc.
- Provide ongoing follow-up services to patients upon discharge from the hospital or during gaps of service provision, including phone contact and face-to-face contact for up to 30 days or as needed during the high-risk gaps clients experience.
- Complete patient documentation detailing the services that have been provided and next steps in the treatment process in a timely manner.
- Maintains appropriate documentation of data pertaining to hospital readmissions, emergency department visits, and follow-up services.
- Track and document monthly outcome objectives.

- Employee maintains current licenses and/or certifications required for the position.
- Practices and reflects knowledge of HIPAA and other federal/state regulatory agencies guiding our organization.
- Completes all annual education and competency requirements as required.
- Is knowledgeable of agency compliance requirements for federally funded healthcare programs (e.g., Medicare and Medicaid) regarding fraud, waste, and abuse. Bring any questions or concerns regarding compliance to the immediate attention of the Corporate Compliance Committee. Takes appropriate action on concerns reported by program/department staff related to compliance.

Education and/or Experience
Pursuing a Bachelor's degree (B.A. or BS) from a four-year college or university in social work or related field or a BSW. (BSW with active IA license preferred)

Language Skills- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Mathematical Skills- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability- Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills- Working knowledge and experience with Excel, Word, email, and overall use of a computer.
To perform this job successfully, an individual should have knowledge of Contact Management systems; Database software; Project Management software; Spreadsheet software and Word Processing software.
Certificates, Licenses, Registrations - Valid Iowa Driver's License. Current Vehicle Registration and Insurance.
Other Qualifications:
Travel -The Case Manager must be able to work in the designated office location and travel as needed within the PAMHC service area as well as to regional and state trainings and meetings, which may require overnight stay.
