Job Summary

We have an exciting opportunity for motivated and dedicated pharmacists to join our inflammatory bowel disease (IBD) care team. The pharmacist will be responsible for the provision of safe and timely pharmacotherapy in the IBD center. Clinical responsibilities include new medication counseling, vaccination management, and protocolized therapeutic drug monitoring overseen by a supervising physician. The pharmacist will liaise with the medical infusion center, specialty pharmacy, and insurers to help provide timely access to biologic and small molecule specialty medications. This includes letters of medical necessity, peer-to-peer reviews, and white bagging decisions. The pharmacist will supervise and receive support from a pharmacy technician specialist who will assist with medication access. The pharmacist will play a key role on the IBD care team through optimization of pharmacy workflows, provision of team education, contribution to institutional formulary medication management related to IBD medications and conduct of quality and process improvements to ensure appropriate clinical metrics are met for patients. They may conduct clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.

The Ambulatory Care Pharmacist will assume responsibility and accountability for the provision of safe and prompt access to medication therapy. Ambulatory Care Pharmacist will proficiently provide patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These pharmacists are aligned with target interdisciplinary programs and specialty services to assist with medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy recommendations and patient education, and providing clinical recommendations to detect, mitigate, and prevent medication related adverse events. They proficiently manage the appropriate use of medications according to professional and regulatory standards. Ambulatory Care Pharmacist will serve as a departmental resource and liaison to other departments, hospital personnel, or external groups. Finally, Ambulatory Care Pharmacist will provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students (MGH Pharmacy Residency website: https://www.massgeneral.org/education/pharmacy-residency.)


 

Qualifications

REQUIRED:

  • Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
  • Postgraduate residency/fellowship training is preferred
  • Board Certification by examination, or equivalent certification, in the relevant specialty is preferred, required within 18 months of hiring
  • Must meet all Board of Pharmacy requirements to enter into a collaborative drug therapy management agreement, when applicable
  • Massachusetts Controlled Substance Registration license, National Provider Index number, and a Drug Enforcement Administration Registration number, when applicable

PREFERRED:

  • Completion of ASHP-accredited PGY1 and PGY2 Internal Medicine, Pharmacotherapy, Ambulatory Care Pharmacy Residency preferred.

Experience
1+ years of experience in a ambulatory clinic setting preferred

Knowledge, Skills and Abilities
- Attention to detail.
- Proficient with standard PC software: Microsoft Excel, Word, PowerPoint.
- Effective computer and keyboarding skills.
- Working knowledge of pharmacy information and inventory systems.
- Familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.
- Ability to manage several priorities and deadlines simultaneously.
