Summary

Help deliver care that meets people where they are and supports where they want to go.Crosstown Clinic is now accepting applications for Licensed Practical Nurses (LPNs) to join their harm reduction-driven team. A leading substance use treatment program in Vancouver’s Downtown Eastside, Crosstown is the first clinic in North America to provide prescription diacetylmorphine through Injectable Opioid Agonist Therapy (iOAT). Here, LPNs provide specialized, relationship-centred care to people with substance dependence, helping clients find greater stability while supporting their goals along the continuum of substance use and connecting them with the community, primary care, mental health and social services they need. Crosstown is supported by an on-site pharmacy and an interdisciplinary care team composed of physicians, nurses, social workers, and other care professionals.

Your duties will include:  

• Provide specialized harm reduction nursing care: Deliver safe, respectful care to clients with substance dependence, monitor their health and wellbeing, administer medications according to established protocols, and respond to clinical incidents and changing needs.

• Support individualized care and continuity: Participate in assessments, care planning and evaluation, contribute to individualized interventions, and help connect clients with community, primary care, mental health and other supports as their needs evolve.

• Build relationships through education and advocacy: Identify client learning needs, provide health education and resources, and advocate for clients’ dignity, rights, autonomy and access to care.

• Collaborate as part of a specialized team: Work closely with nurses, physicians, social workers and other care professionals, contribute to care conferences and team planning, support orientation of new staff and students, and contribute to a safe, effective clinical environment.

Minimum Nurse-to-Patient Ratio

Minimum Nurse-to-Patient Ratios (mNPR) set the minimum number of nurses required to safely care for patients on a given unit. Developed in partnership with the Ministry of Health, the BC Nurses' Union (BCNU), and health organizations across British Columbia, mNPR provides a consistent and transparent approach to staffing that supports safe nursing practice and high-quality patient care. What You Bring

Education: 

• Graduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience.
• Registration with the British Columbia College of Nurses and Midwives as a practicing LPN registrant.
• Current basic cardiac life support certification.

Skills & Abilities:  

• Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service
• Physical ability to carry out the duties of the position
• Ability to apply decision making and analytical skills
• Ability to role model conflict resolution strategies
• Ability to prioritize and organize work
• Ability to teach and identify patient/resident learning needs

Your day to day: 

• Provide safe and respectful nursing care, adhering to established policies, individualized care plans, and priorities.
• Conduct thorough assessments, monitor signs/symptoms, and communicate with the RN/RPN as needed.
• Prepare patients for procedures, transfers, and admissions, coordinating with the interdisciplinary team.
• Administer medications per protocol, monitor effects, and maintain communication with the team.
• Utilize evidence-based resources to stay updated on nursing practices.
• Respond to incidents such as codes, injuries, spills, and alarms, following procedures.
• Assist with staff orientation, demonstrating procedures and providing support.
• Address patient/resident and family learning needs, provide educational materials, and make referrals.
• Attend care planning meetings, provide input, and advocate for patients/residents.
• Collaborate with the team to develop and evaluate individualized care plans.
• Assist with discharge planning based on patient status and external supports.
