Job Description:
Seeking energetic, team-oriented Discharge Planner for meaningful work with people admitted to our inpatient state designated receiving facility. We provide short-term crisis stabilization and brief 1:1/group counseling in a milieu setting.
Requirements
Key Responsibilities:
Working alongside a busy inpatient psychiatric unit to coordinate patient discharge appointments. Mental health clinical experience required, as this position involves working closely and collaboratively with patients on their plans for discharge and goals for overall treatment. Must be a strong communicator and highly organized. We are looking for a candidate who will initiate tasks and communicate effectively to a large team. The Discharge Planner will have some knowledge of mental health and substance misuse resources in NH and will be committed to building ongoing relationships with collateral partners. We are seeking someone who is highly reliable, focused, and experienced in customer service. May be involved in several committees within the organization to enhance relationships and strengthen knowledge of available resources
Experience:
- At least one full year working in the mental health field is required.
- Preference is given to previous experience working in crisis units/environments.
Education: A bachelor’s degree in psychology or a related field is required.
