Case Manager/Discharge Planner

Under limited direction coordinates the plan of care for the patient and family through the continuum of care, in accordance with the CVRMC Clinical Resource Management Plan.

QUALIFICATIONS:

• Must have either a current Arizona RN license or Clinical Social Work certificate in good standing, BSN or CSW preferred
• Must have at least 5 years of clinical experience.
• Recent case management experience in a healthcare setting preferred.
• Certification in Case Management by a recognized national Board preferred, and will be required within three years of hire.
• Must be able to interact effectively with co-workers, patients/family, physicians, and other hospital personnel.
• Must be able to perform effectively under physical and mental stress, maintaining concentration during that duration.
• Maintains current Case Management certification ( once acquired), maintains CPR.
