Full-Time | Days Shift: 7:00am-3:30pm/Every other weekend rotation required

 Location: 2404 Valparaiso Street, Valparaiso, IN 46383

 6-Bed Hospice Inpatient Unit

Join Our Compassionate Hospice Team

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Hospice Inpatient Care Center team. This position provides skilled nursing care to patients receiving end-of-life care in our intimate 6-bed inpatient hospice unit. The ideal candidate is passionate about delivering exceptional patient-centered care while supporting patients and their families during life's most meaningful moments.

Position Summary

The Hospice Center RN is responsible for providing comprehensive nursing care to hospice patients with complex symptom management needs. The RN works collaboratively with physicians, advanced practice providers, social workers, chaplains, hospice aides, and other interdisciplinary team members to ensure comfort, dignity, and quality of life for patients and families.

Essential Responsibilities

  • Assess, plan, implement, and evaluate individualized patient care plans.
  • Manage pain and symptoms utilizing hospice and palliative care best practices.
  • Administer medications, treatments, and therapies in accordance with physician orders.
  • Monitor patient condition and communicate changes to the hospice interdisciplinary team.
  • Provide education and emotional support to patients and their families.
  • Collaborate with physicians and team members to coordinate patient care.
  • Maintain accurate and timely documentation in the electronic medical record.
  • Advocate for patient comfort, dignity, and end-of-life wishes.
  • Participate in team meetings, care conferences, and quality improvement initiatives.
  • Provide compassionate support to families throughout the hospice journey.

Requirements


Qualifications


Required:

  • Current Indiana Registered Nurse (RN) license in good standing.
  • Graduate of an accredited nursing program.
  • Current CPR certification.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Excellent communication and critical thinking skills.

Preferred:

  • Previous hospice, palliative care, oncology, critical care, medical-surgical, ER, ICU or long-term care experience.
  • Experience with symptom management and end-of-life care.
