Position Summary
Trinity Health Alliance of Michigan (THAM) is seeking a highly motivated and relationship-focused Practice Coach - Office Specialist to partner with physician practices across our clinically integrated network. This highly visible role serves as a trusted advisor to physicians and office staff, helping practices improve quality outcomes, patient experience, operational efficiency, and value-based care performance.
The Practice Coach - Office Specialist is responsible for educating, training, coaching, and supporting provider offices in the implementation of clinical integration initiatives, alternative payment model (APM) programs, credentialing activities, and practice transformation strategies. Through data analysis, performance coaching, and process improvement, this role helps providers achieve excellence in quality, total cost of care, and the Quadruple Aim goals.
This position requires strong collaboration skills, healthcare operational expertise, and the ability to influence change within physician practices while maintaining positive relationships with network participants.
Key Responsibilities
Practice Transformation & Clinical Integration
- Support THAM performance initiatives focused on quality improvement, total cost of care, and patient outcomes.
- Conduct medical office assessments, chart audits, and data collection activities to identify performance improvement opportunities.
- Educate physicians and office staff on clinical integration initiatives, quality programs, value-based care requirements, and performance expectations.
- Interpret and explain performance reports, quality scorecards, and clinical integration metrics to providers and practice teams.
- Develop educational presentations and communication materials for physician offices and THAM leadership.
- Perform physician facility site audits and maintain audit tracking databases and documentation.
- Investigate and resolve provider and member concerns related to access to care, compliance issues, and reimbursement matters.
- Support provider credentialing activities and ensure compliance with participation agreement requirements.
Network Engagement & Provider Coaching
- Conduct regular in-person and virtual coaching sessions with physician practices to review performance metrics and develop improvement plans.
- Collaborate with physicians, practice leaders, Care Managers, and Pharmacists to improve quality, efficiency, risk adjustment performance, and cost outcomes.
- Develop actionable recommendations that support achievement of clinical integration, APM, and quality program goals.
- Monitor progress against performance improvement plans and provide ongoing consultation and support.
- Educate providers on performance scoring methodologies, reporting systems, attribution methodologies, registries, and THAM-supported tools.
- Partner with Medical Directors and Network Development leadership to support physicians requiring focused performance improvement.
Data Analysis & Performance Management
- Analyze practice and population health data to identify improvement opportunities.
- Develop, implement, and maintain monitoring tools and tracking mechanisms to support provider action plans.
- Maintain accurate records of physician interactions, follow-up activities, and engagement efforts.
- Assist in developing educational materials, newsletters, decision-support resources, alerts, and other communication tools that support network goals.
Network Operations & Education
- Train physicians and office staff on THAM-supported technology platforms, quality reporting systems, and network resources.
- Educate providers on referral protocols, clinical guidelines, ancillary service utilization processes, and network requirements.
- Assist physician offices with system access, account setup, passwords, and administrative support.
- Serve as a liaison between provider offices and payors when support is needed regarding network contract concerns.
- Coordinate and deliver onboarding, education, and training programs for new and existing providers.
- Promote and disseminate best administrative and clinical practices across network practices.
- Collaborate with Physician Relations Consultants and other stakeholders to support provider education and engagement initiatives.
Compliance & Additional Responsibilities
- Maintain knowledge of applicable federal, state, and local regulations, Trinity Health policies, and the Integrity and Compliance Program.
- Ensure all activities are conducted in accordance with ethical, professional, and regulatory standards.
- Support special projects and perform additional duties as assigned.
Qualifications
Required Education & Experience
- Certified Medical Assistant or Associate's Degree from an accredited college or university.
- Minimum of five (5) years of progressive healthcare experience working directly with physicians, physician practices, and healthcare leaders.
- Demonstrated knowledge of:
- Practice transformation
- Clinical integration networks
- Population health management
- Patient-Centered Medical Home (PCMH) models
- Quality improvement methodologies
- Value-based care and alternative payment models
Preferred Qualifications
- Bachelor's Degree in Healthcare Administration, Health Management, Business Administration, or related field.
- Experience in practice coaching, provider engagement, or healthcare consulting.
- Familiarity with change management methodologies and facilitation techniques.
- Experience with quality reporting systems, healthcare analytics, and provider performance improvement programs.

- Excellent presentation, facilitation, and coaching abilities.
- Ability to analyze complex clinical and operational data and translate findings into actionable recommendations.
- Knowledge of value-based care, risk adjustment, quality measures, and population health strategies.
- Proficiency with Microsoft Office Suite and healthcare information systems.
- Strong organizational, project management, and follow-through skills.
- Exceptional written and verbal communication skills.
- Ability to work independently while effectively managing multiple priorities and travel between provider offices.
