Home Health Clinical Supervisor (RN)
Full-Time | Braintree, MA

Your Impact
You’ll be the clinical resource your team can count on. Your responsibilities will include:
Overseeing patient care, clinical staffing, referrals, and assignments
Ensuring timely starts of care and seamless care coordination
Reviewing plans of care, physician orders, and clinical documentation
Conducting OASIS reviews to support accuracy, quality, and compliance
Monitoring patient needs and ensuring care plans remain appropriate
Coaching clinicians and providing ongoing education and feedback
Supporting recruiting, interviewing, onboarding, and staff development
Partnering with physicians, patients, families, providers, and vendors
Monitoring key clinical and operational performance metrics
Providing field support when needed
What You Bring
Current, unrestricted Massachusetts RN license
Associate degree in nursing
At least two years of RN experience in home health or hospice
Supervisory or clinical leadership experience preferred
Strong OASIS, care coordination, and clinical documentation knowledge
A natural ability to coach, communicate, and bring out the best in others
Strong organization, attention to detail, and clinical judgment
Ability to learn systems such as Homecare Homebase and Workday
Dependable transportation, a valid driver’s license, and current auto insurance
Willingness to travel within the branch’s coverage area
Ability to sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
