Clinical Scheduling & Compliance Coordinator – RN

Grace Hospice is looking for a Clinical Scheduling & Compliance Coordinator – RN to join our Quality and Compliance team. In this role, you won’t manage a traditional patient caseload. Instead, you’ll use your nursing knowledge to support clinical scheduling, coordinate important follow-up, identify potential gaps, and help our care teams stay organized and on track.
You’ll work closely with clinical leadership and our interdisciplinary hospice team, making an impact on patient care by helping ensure the right services happen at the right time.

Reports To: Director of Quality and Compliance
License Required: Current Minnesota RN license
Pay Range: $86,000–$90,000 annually, depending on experience
What You’ll Do
- Coordinate clinical schedules and visit assignments across hospice disciplines.
- Monitor open, missed, or overdue visits and help ensure timely follow-up.
- Track required clinical visits, assessments, documentation, and other important deadlines.
- Enter authorized discipline orders and help ensure schedules align with current plans of care.
- Identify scheduling, documentation, or compliance concerns and help coordinate timely resolution.
- Partner with clinical leaders and team members to keep workflows organized and patient care moving forward.
- Support audits, quality initiatives, survey readiness, and QAPI activities.
- Serve as a resource for clinical staff related to scheduling and electronic health record workflows.
- Help identify opportunities to improve processes, communication, and coordination across the clinical team.
What Will Help You Succeed
- Current, active Minnesota RN license.
- Two or more years of nursing or related healthcare experience preferred.
- Hospice, home health, clinical coordination, or similar experience is a plus.
- Experience working with electronic health records and scheduling systems; MatrixCare experience preferred.
- Strong organizational skills and attention to detail.
- Ability to prioritize, follow through, and keep multiple moving pieces on track.
- Excellent communication, critical-thinking, and problem-solving skills.
- A collaborative approach and a willingness to support others.
- Comfort identifying concerns, asking questions, and helping find solutions.
