Description
What You’ll Be Doing:
- Evaluate balance, gait, strength, range of motion, and functional mobility in the home environment.
- Identify environmental and physical barriers that may affect safe mobility and independence.
- Develop and implement individualized treatment plans to improve or maintain mobility, strength, balance, and functional independence.
- Provide rehabilitation for patients recovering from illness, injury, surgery, or managing chronic conditions.
- Train patients in safe transfers, ambulation, stair negotiation (when applicable), and household mobility.
- Educate patients and caregivers on fall prevention, safe movement strategies, and the use of assistive devices.
- Monitor patient progress, reassess functional status, and modify treatment plans based on clinical findings.
- Collaborate with physicians and the interdisciplinary care team to support patient goals.
- Complete timely documentation using the agency's electronic medical record (EMR) system.
- Maintain compliance with clinical, regulatory, and agency documentation standards.
- Flexible Scheduling: Choose your availability and manage your own caseload.
- Suburban Community Caseload: Provide care across Farmingdale and surrounding Long Island communities.
- Operational Support: Scheduling and administrative coordination handled by the office team.
- Scalable Workload: Ability to increase or adjust caseload based on preference.
- Active New York State Physical Therapist license.
- Experience with adult or geriatric rehabilitation preferred.
- Strong understanding of functional mobility and balance training.
- Ability to work independently in a home care setting.
- Strong communication and clinical decision-making skills.
