Registered Nurse Case Manager (Field) – Hospice | Honolulu, HI (Oahu)
We are partnering with a well-established hospice organization serving Oahu, Hawaii to find a compassionate and experienced Registered Nurse Case Manager to join their dedicated interdisciplinary hospice team. This is a full-time, field-based opportunity for an RN who is passionate about end-of-life care and committed to promoting comfort, dignity, and quality of life for patients and families during one of life's most meaningful transitions.
Position Details
- Setting: Hospice — Field-based (patient homes, assisted living facilities, skilled nursing facilities, and inpatient hospice units across Oahu)
- Patient Population: Terminally ill adults and their families — community-based hospice and palliative care
- Schedule: Full-Time | Monday – Friday | 8:00 AM – 5:00 PM
- On-Call: Rotating on-call coverage required per agency policy
- Location: Oahu, Hawaii — must reside on Oahu
- Pay Rate: $43.00 – $50.00 per hour

About the Role
The RN Case Manager plans, organizes, and directs hospice care, providing skilled nursing and case management services to terminally ill patients and their families in accordance with Medicare Conditions of Participation (42 CFR Part 418), state regulations, and agency policies. Working as part of a collaborative interdisciplinary team, you will be the clinical anchor for your caseload — coordinating care, managing symptoms, supporting families, and ensuring every patient receives compassionate, individualized end-of-life care that honors their goals and values.
Key Responsibilities
Patient Care & Clinical Assessment
- Complete comprehensive initial, ongoing, and as-needed assessments of patients and families — including physical, psychosocial, emotional, spiritual, and environmental factors
- Provide skilled nursing care in accordance with the Hawaii Nurse Practice Act and hospice standards of practice
- Assess and manage pain and symptoms using evidence-based hospice and palliative care practices
- Develop, initiate, and regularly re-evaluate individualized plans of care reflecting patient goals and hospice philosophy
- Implement physician orders and administer medications and treatments as prescribed
- Provide wound care, medication management, symptom control, and end-of-life care
- Recognize changes in patient condition and initiate timely and appropriate clinical interventions
- Support patients and families during the active dying phase and at the time of death
Case Management & Care Coordination
- Act as Case Manager for an assigned caseload — coordinating care across all settings including private homes, ALFs, SNFs, and inpatient hospice units
- Ensure continuity of care through effective communication with physicians, patients, families, caregivers, and interdisciplinary team members
- Participate in interdisciplinary group (IDG) meetings and contribute to collaborative care planning and goal setting
- Ensure appropriate durable medical equipment, medications, supplies, and services are in place
Interdisciplinary & Supportive Care Collaboration
- Identify psychosocial, emotional, spiritual, and bereavement needs and communicate findings to the IDG
- Coordinate referrals to social work, chaplaincy, bereavement, and volunteer services based on patient and family needs
- Provide culturally sensitive care respecting diverse cultural, spiritual, and family traditions
Patient & Family Education
- Educate patients and families on disease progression, symptom management, medications, and end-of-life care
- Provide anticipatory guidance, emotional support, and reinforcement of hospice goals of care
- Teach self-care techniques to patients and caregivers as appropriate
Documentation & On-Call
- Complete accurate, timely, and compliant clinical documentation in the electronic medical record
- Participate in rotating on-call coverage; respond to urgent patient needs, symptom crises, and death pronouncements per agency policy
Requirements
- Graduate of an accredited school of nursing — required
- Current Hawaii RN license in good standing — required
- Current BLS/CPR certification — required
- 1–2 years of recent acute care experience in an institutional setting — required
- 1–2 years of recent home care experience — required
- Bachelor's degree with 1 year of hospice care experience — preferred
- Must reside on Oahu — required
- Valid driver's license with insured, reliable vehicle in good working order — required
- Mileage reimbursement provided
- Strong observation, communication, problem-solving, and clinical assessment skills
- Ability to supervise hospice aides
