Description

Aging and Disability Services, a division of the Human Services Department, has received Veteran, Seniors and Human Services Levy funds to hire a Multidisciplinary Team Coordinator for the Seattle-King County Elder Abuse Multidisciplinary Team (MDT). We are seeking a dynamic, innovative planner to join our team. This multidisciplinary team is a powerful, person-centered intervention that brings together professionals from across disciplines and systems to respond to complex cases of elder abuse. The team (comprised of prosecutors, a detective, APS investigators, ADS case managers, a victim advocate, a geriatrician and a forensic analyst, among others) will accept cases involving elder and vulnerable adult abuse, neglect, self-neglect and financial exploitation throughout King County.

As our ideal team member, you will have a deep understanding of the range of providers who interact with older adults. You are enthusiastic about this job opportunity and have demonstrated interest in issues pertaining to elder abuse and aging. You will also commit to thinking and acting “outside the box” to support the MDT and bring this team to the next level. This role is highly visible and requires well-developed facilitation and presentation skills.

You will have experience in project management and evaluation, data collection and reporting, and clinical expertise in working directly with older victims and/or their family members. You have strong boundaries and are experienced in handling sensitive, confidential case material.

The MDT Coordinator’s role will primarily be to:

a) Facilitate team meetings and oversee all operations of the team, including all reporting requirements for data collection and evaluation;

b) Coordinate outreach and training to the general public, medical and social service providers, law enforcement and other first responders; and

c) Serve as a link to professionals and the public.

The MDT Coordinator will co-locate between the King County Prosecuting Attorney’s Office located within the King County Courthouse and ADS offices and report to a Human Services Manager.

  • Coordinate the day-to-day operations of Seattle-King County Elder Abuse MDT activities.
  • Field calls from professionals referring cases to the MDT, assist with completion of intake form, and determine whether case is appropriate for staffing.
  • Review cases screened out by APS on a regular basis; contact appropriate law enforcement agency, make referrals for services and to MDT, and reach out to victims as necessary.
  • Coordinate case presentations and facilitate MDT meetings (twice monthly at a minimum).
  • Follow up on cases that have been presented for MDT staffing and coordinate additional follow-up and meetings as necessary.
  • Represent the MDT and its partners in building relationships with law enforcement, the medical community, the legal community, social service entities and other relevant agencies in King County. Coordinate proactive outreach and training to build a comprehensive network of community, professional, banking, and government organizations to increase and improve collaborative referrals, consultations, and victim services.
  • Outreach proactively to priority populations to increase and improve referrals and victim services.
  • Collect, track and report data, including case data from MDT discussions, consultations, outreach, and contacts developed. Ensure the MDT’s compliance with all reporting requirements.
  • Track budget, collect data, and assist with MDT authorizations related to client flexible fund. Ensure the MDT’s compliance with all reporting requirements.
  • Synthesize information gathered and data collected from cases reviewed to determine gaps in systemic response to reports of abuse, neglect, and exploitation, and make recommendations to ADS and PAO for MDT resource allocation and training.
