- Conduct occupational therapy evaluations and provide treatment within patients’ homes
- Create personalized care plans designed to promote independence
- Instruct patients and caregivers on safety techniques, adaptive methods, and equipment use
- Complete documentation in a timely and accurate manner
- Share updates on patient progress with the care team across disciplines
- Ensure adherence to all clinical guidelines and documentation requirements
- Active Occupational Therapist (OT) license
- Prior home health experience is a plus, but not mandatory
- Excellent verbal and written communication abilities
- Capable of working autonomously while effectively managing your schedule
