- Education: Associate’s or Bachelor’s degree in Nursing (ADN/BSN).
- Licensure/Certifications:
- Active New York RN license.
- MDS Assessor Certification (required).
- Experience: 1–2 years in long-term care with MDS coordination experience.
- Soft Skills: Attention to detail, strong communication, and ability to lead interdisciplinary teams.
- Intake and evaluation process of patients in long-term care facilities.
- Up-to-date assessments of the patient and oversee the interdisciplinary assessment for all nursing home patients.
- MDS coordinator monitors patient care by assessing procedures.
- Speaking with patients and recording medical codes.
- MDS coordinators also enter medical codes and are involved in medical billing processes, ensuring they maintain accurate medical records.
