Supported Living Facilitator - Planning and Admin | QCSS  

  • Employment Term: Permanent Full Time 

  • Location: Annerley  

  • Remuneration: $34.58 p/hr + Super + NFP Salary Packaging (save tax and increase your take home pay)

Your part in our UnitingCare team 

You’ll support people living with disability to live meaningful lives through person-centred planning, community connection and coordinated administrative support. The role contributes to the delivery of high-quality, compliant services by facilitating individual plans, supporting daily living outcomes, and undertaking administrative and record-keeping functions that enable effective service delivery. You will:  

  • Provide support to people in their everyday living situations, including personal care as identified, with a strong emphasis on choice, enhancing quality of life, health and wellbeing, whilst ensuing duty of care is always upheld 

  • In conjunction with program leadership – develop, implement, and review individual plans of the person being supported.  

  • Assist people to fulfil valued community roles through engagement with community groups, potential employers and suitable programs or activities. 

  • Advocate for and facilitate warm referrals to suiable services to address client needs outside the scope of the program.  

  • Undertake a range of administrative functions that directly support service delivery and individual outcomes, including: basic financial and transactional activities, data capture, documentation and reporting related to case management, monitoring and supporting compliance with organisational, funding and legislative requirements.  

  • Undertake other administrative duties such as reception back up when required 

  • Contribute to an environment of excellence and drive for positive outcomes

  • Able to work effectively with others in a team-oriented environment and provide excellent support to clients and co-workers 

  • Able to effectively communicate in spoken and written English 

  • Relevant certification and demonstrated experience in the Disability sector 

  • Awareness of legislation underpinning the provision of services within the Disability sector and the key issues affecting quality of life for people with a Disability 

  • A current NDIS Worker screening check  

  • Current CPR and First Aid Certificate (required before you start)
