Halcyon Home is hiring a Hospice LMSW PRN to support our growing geriatric Hospice population in Katy, TX and surrounding areas. We offer competitive pay, and wonderful team support.

Responsibilities:
- Perform hospice social work visits in assisted living, independent living communities, skilled nursing facilities, and residences for hospice patients within Houston area.
- Conduct comprehensive assessments of patients' social and emotional needs
- Assist with coordination of care and access to community resources
- Maintain detailed and accurate documentation of patient interactions and progress
- Utilize Homecare Homebase EMR (knowledge of which is a plus)
- Develop personalized care plans in collaboration with the interdisciplinary team
- Provide counseling and support to patients and families facing end-of-life challenges
Requirements
- Licensed as an LMSW
- Knowledge or experience utilizing Homecare Homebase EMR would be a plus
- Minimum of one year of hospice care experience
- Minimum of one year of social work experience
- Compassionate and empathetic approach to working with the elderly
- Strong clinical and assessment skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage time effectively
Payrate: $55-$75 per visit
