Position Overview:

The Stroke Nurse Navigator is a professional Registered Nurse (RN) who provides individualized guidance, education, coordination, and support to stroke patients, families, and caregivers across the continuum of care. This role serves as a key resource within a Certified Comprehensive Stroke Center, utilizing specialized knowledge and experience in the care of patients with strokes to improve patient outcomes and reduce barriers to care.
The Stroke Nurse Navigator enhances the patient experience and promotes optimal clinical outcomes through seamless care coordination from acute hospitalization through rehabilitation, outpatient follow-up, and community reintegration. Responsibilities include providing comprehensive inpatient stroke education to patients and families, reinforcing stroke prevention strategies, risk factor modification, medication adherence, and recognition of stroke warning signs. The role also leads and participates in community outreach initiatives focused on stroke awareness, prevention, education, and access to resources.
The Stroke Nurse Navigator participates in the development, implementation, monitoring, and sustainability of stroke program initiatives, quality improvement activities, regulatory compliance efforts, and performance measures associated with maintaining Comprehensive Stroke Center certification. Through collaboration with inpatient, outpatient, rehabilitation, and community partners, the navigator works to improve transitions of care, support evidence-based stroke management, and promote positive outcomes for patients, providers, and the health system. Certified Neuroscience Registered Nurse (CNRN) or Stroke Certified Registered Nurse (SCRN) certification preferred.

Job Description:

EDUCATION:
Other: Graduate of an accredited school of nursing.

CERTIFICATION & LICENSURE:
RN-Registered Nurse of California
BLS-Basic Life Support Healthcare Provider

TYPICAL EXPERIENCE:

5 years recent relevant experience.


SKILLS AND KNOWLEDGE:
Knowledge of medical terminology required.
Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff.
Knowledge of third-party reimbursement helpful.
Verbal and written communication skills.
Microsoft Office software experience required, including Excel and Word

Pay Range is $95.11 to $133.15 / hour

The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
