Registered Nurse – Home Health

Full-Time | Monday–Friday | Chicopee–Springfield Territory

What You’ll Do

As a Home Health RN, you will:

• Manage an assigned caseload of home health patients

• Complete comprehensive assessments and ongoing evaluations

• Develop, implement, and update individualized plans of care

• Coordinate services with physicians, therapists, caregivers, and clinical leadership

• Educate patients and families on medications, treatments, disease management, and safety

• Monitor changes in each patient’s condition and communicate significant findings

• Maintain accurate, timely clinical documentation

• Help patients achieve better outcomes while remaining safely at home

What You’ll Need

• Current, unrestricted RN license

• At least one year of clinical nursing experience

• Associate degree in Nursing required; bachelor’s degree preferred

• Previous home health or hospice experience preferred

• Homecare Homebase experience preferred

• Dependable vehicle, valid driver’s license, and current auto insurance

• Ability and willingness to travel throughout the assigned territory

• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
