Physical Therapist - Exeter Hospital

Plans and administers medically prescribed physical therapy evaluation and treatment for patients suffering from injuries, muscle, nerve, joint and bone diseases, or neurological or developmental disorders to improve and/or restore function, relieve pain and prevent disability.

Job Description:

• $10,000 sign-on bonus available for eligible hires. Bonus amounts for positions under 40 hours per week will be prorated. Per diem roles are not eligible.

Key Responsibilities:

• Select, administer, and interpret clinically appropriate assessments.
• Integrate educational, medical, social, and psychological factors when evaluating patients and determining treatment diagnoses.
• Develop and implement individualized plans of care that promote skill development, functional independence, and successful participation in daily and community activities.
• Collaborate with healthcare providers, contracted agencies, schools, and other care team members to coordinate services and maximize patient outcomes.
• Evaluate treatment progress and outcomes and modify plans of care as needed based on changes in patient condition or performance.
• Design and implement home exercise programs to support carryover of skills and promote patient independence; provide education and supervision to patients and caregivers.
• Maintain accurate and timely documentation, including treatment notes, insurance authorizations, referrals, consents, and other required medical or educational records.
• Ensure compliance with HIPAA regulations, patient confidentiality standards, professional ethics, and licensure requirements.
• Maintain a safe, supportive, and therapeutic environment for patients and families while upholding patient rights and responsibilities.
• Participate as a collaborative member of the interdisciplinary care team to deliver high-quality, patient-centered care.

Qualifications:

• Bachelor's degree; graduate of accredited program
• Physical Therapist – NH
• CPR certified within 90 days of hire

 

 

Pay Range:

$34.53 - $56.63

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
