Hospice and Palliative Care Buffalo is hiring Registered Nurses (RNs) to join our growing clinical team. We currently have opportunities in home care, facility-based care, community admissions, and on-call nursing.

Current Registered Nurse (RN) Opportunities

We have several full-time Registered Nurse positions available in Buffalo. Opportunities and schedules include:

Hospice Home Care Registered Nurse (RN) Monday–Friday | 8:00 AM–4:00 PM | Holiday rotation

Provide hospice and palliative nursing care to patients in their homes, helping them maintain their independence and quality of life while receiving care in a familiar environment.

Facility-Based Services Registered Nurse (RN) Monday–Friday | 8:00 AM–4:00 PM | Holiday rotation

Provide individualized hospice nursing care and support to patients and families in facility-based settings.

Community Admissions Registered Nurse (RN) Four 10-hour shifts | 9:00 AM–7:00 PM | Weekend and holiday rotation

Meet with patients and families referred for hospice services, assess their needs and goals of care, determine the appropriate level of hospice care, and facilitate the admission process.

On-Call Registered Nurse (RN) – Evenings Monday, Wednesday, Thursday and Friday | 2:00 PM–12:00 AM | No weekends | Holiday rotation

Provide responsive hospice nursing care to patients and families during evening hours in home and facility-based settings.

On-Call Registered Nurse (RN) – Weekends Saturday and Sunday | 8:00 AM–8:00 PM | Plus one 8-hour weekday shift | Holiday rotation

Provide hospice nursing support to patients and families during weekend and after-hours periods in home and facility-based settings.

Responsibilities

Responsibilities will vary depending on the Registered Nurse position and care setting and may include:

  • Provide compassionate, individualized nursing care to hospice and palliative care patients.
  • Perform comprehensive assessments addressing physical, psychosocial, environmental, safety, emotional, and spiritual needs.
  • Assess and manage pain and other symptoms associated with serious and terminal illness.
  • Develop, initiate, and update individualized plans of care based on patient needs.
  • Recognize changes in patient condition and provide timely and appropriate clinical interventions.
  • Educate and support patients, families, and caregivers.
  • Collaborate with physicians and interdisciplinary team members to coordinate patient care.
  • Help patients and families understand hospice services and appropriate levels of care when applicable.
  • Maintain accurate and timely electronic documentation.
  • Follow established clinical, patient safety, privacy, and infection-control standards.
  • Travel to patient homes and/or healthcare facilities as required by the position.

Qualifications

  • Graduate of an accredited school of nursing.
  • Current Registered Nurse (RN) license in the State of New York.
  • Valid driver's license and access to a vehicle for positions requiring community travel.
  • One year of professional nursing experience preferred for most Registered Nurse opportunities.
  • Two years of professional nursing experience preferred for the Community Admissions Registered Nurse position.
  • Certification in Hospice and Palliative Care Nursing preferred for applicable positions.
