What You’ll Do:
- Perform patient assessments and deliver direct patient care under the supervision of a Registered Nurse
- Assist with medications and treatment orders per patient’s individualized plan of care
- Complete accurate documentation as part of maintaining patient records
- Communicating status updates with ordering providers and Care Teams
- Educate patients and Families on management of disease processes
- Active Unencumbered Texas LVN License
- Minimum of 1 Year recent experience as a licensed nurse (Prior Home Health Experience is Preferred)
- Strong Clinical and Communication Skills
- Current valid CPR Certification including hands-on skills component (i.e. American Heart Association or American Red Cross)
- Negative TB test completed within the last 12 months
