Our holistic, evidence-based model offers a wide range of services - therapy, case management, yoga, behavioral health and substance use recovery groups, spiritual care, life skills, medical care, education, and job readiness. Each participant is formally supported by our Care Team for a full year, ensuring stability and long-term success. Position Overview: Post-Release Therapist:
The post-release therapist supports participants on their healing journey. Working alongside a comprehensive care team, you will employ our healing-first approach and help participants create and achieve the goals in their treatment plan. 


Your role will involve:

Key Responsibilities: 
  • Facilitating excellent participant programming: 
    • Conducting impactful weekly 1:1 and group sessions with participants utilizing cognitive behavioral therapy and motivational interviewing approaches to support participants in achieving their goals 
    • Providing frequent, supportive communications to participants throughout post-release phases
    • Updating participants’ objectives and goals in their treatment plans 
    • Using assessments to confirm treatment plan goals and progress 
    • Upholding the quality and fidelity of the Concordance Re-entry Model curriculum when delivering 1:1 and group sessions 
    • Preparing for sessions in advance to maximize effectiveness of interactions 
    • Evaluating participant progress and support needed, with the use of assessments and other evaluations as needed 
  • Communicating with the multidisciplinary care team: 
    • Documenting sessions (for individuals and groups), including noteworthy participant interactions in the electronic record system 
    • Working collaboratively with participants’ care teams, providing input on progress and flagging holistic participant needs 
    • Participating in internal meetings (Behavioral Health and Wellness team meetings, all team meetings, 1:1 supervisor meeting(s), and training sessions). Occasionally, help set up for center events or move the classroom around, including but not limited to carrying and setting up tables and chairs.

Required Education and Licensure: 
  • Master’s degree in counseling, social work, behavioral health, or related field 
  • Independent clinical license 
Skills and Competencies: 
  • 3-5 years of experience working in mental health and substance use disorders, with equal focus on mental health and substance use treatment 
  • Familiarity with and endorsement of a 12-step program and equal experience with traditional cognitive behavioral therapy interventions 
  • CPR / Adult First Aid certification preferred

Physical Demands:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 
  • This person must be able to sit or stand at a personal computer for typing and reading. This person must be able to see, hear, speak, walk, and sit. 
  • Must be able to use hands and fingers to handle or feel objects, tools, or controls; reach with hands and arms; stoop, kneel, crouch, or crawl. 
  • Vision abilities required for this job include close vision, distance vision, peripheral vision, depth perceptions, and the ability to adjust focus. 
  • Occasionally, help set up for center events or move classrooms around, including but not limited to carrying and setting up tables and chairs.
