TITLE: Home Health RN Case Manager (RNCM) + Recertification Nurse

LOCATION: North-Central Ohio inc. Erie, Huron, Sandusky and adjacent Counties (1-hour travel radius required)

Home Care Network is expanding and actively seeking skilled an experienced RN Case Manager / Recertification Nurse interested in building a long-term employment relationship with our agency by consistently accepting and executing RNCM and recertification visits and weekly medicine setups as needs arise, and providing in-office State Survey support as needed (1-2x/year) in our Perrysburg/Toledo Office, under the guidance of our Northwest Ohio Administrator.

  • There is potential for growth into a Full-Time RNCM position as our agency grows over the next 6-12 months, though we cannot guarantee additional hours at this time.
  • RNs with nights-only and weekends-only availability cannot be considered at this time.
  • Because of the flexible schedule, many nurses choose to pair this opportunity with other employment, and we're happy to work around existing commitments so long as you are able to accept most / all of our client care requests and support State Surveys.

This role requires a mix of RNCM services and recertification visits - and travel - as needed to all clients across the far-West Northwestern Ohio region.

  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NW Ohio and our current + prospective employees in these communities.

Service Area

Priority Region: North-Central Ohio (incl. Sandusky, Bellevue, Norwalk and surrounding areas) with 1-hour travel radius required

  • HCN requires typical travel distances up to a 1-hour radius.
  • Willingness to support occasional travel up to 2 hours is highly desired (not required)
  • Additional hours are available with expanded travel radius if desired (not required).

MINIMUM REQUIRED SERVICE LOCATIONS INCLUDE:

  • Erie County
  • Huron County
  • Sandusky County
  • Adjacent counties as needed

Friendly Travel Expectations

  • For PT/FT Positions: Willingness to travel to and from all client residences to provide care during scheduled, agreed-upon dates and times is required.
    • Your service area is based upon client-specific locations and your acceptance to support them.
    • Willingness to cover clients up to a 60-minute radius is required, with willingness to support expanded travel as able and as needed (up to 2 hours) highly desired (not required).
  • For PRN Shifts / Positions: up to a 60-minute radius (from your home) is required, and willingness to cover clients up to a 2 hour radius is highly desired.
  • Interested in Nursing as a "Road Warrior"?
    • Travel is required as needed to all clients throughout North-Central Ohio (approx. 2-hour radius service area).
    • Typical travel distances include a 60-minute radius daily + up to 2 hours on occasion.
    • Door-to-Door mileage reimbursement provided.
    • Qualified applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.

Compensation

  • Part-Time RNCM Hourly Wage: $37 - $40/hour + door-to-door mileage reimbursement

  • All positions at HCN are long-term employment opportunities (2+ years desired, or longer) requiring commitment to a part-time, recurring weekly schedule.
  • No Weekend requirements / No Overnight travel required.

Part-Time Positions

Whether you're seeking a small or large Part-Time schedule (5-25 hours/week), or starting at Part-Time with a desire to build up a Full-Time schedule (32-40 hours/week), HCN RNCMs have opportunities to:

  • Create flexible schedules based on daily/weekly availability
  • Specify available days & hours per day (+ blackout days/times) from which we build your schedule
  • Ease in Occasional flexible scheduling to accommodate holidays / personal commitments of employee and client

Whether assigned to care for existing HCN clients, new HCN clients, family member clients, or to support the continuity of care for past clients (when appropriate), we understand the importance of trusted caregiver-client relationships and create smooth transitions and lasting, predictable schedules for both caregivers and clients.

  • For those seeking consistent hours weekly: Qualified nurses who are interested in ongoing employment with increased hours (Part-Time or Full-Time) may be required to start with immediate-need clients and build up to desired total hours/schedule within 60 days. HCN will seek available new cases on your behalf, and you will have first priority for any existing or new cases that arise in your target geography

PRN Opportunities

Whether you're solely seeking a PRN / As-Needed position or are interested in additional hours beyond your Part-Time or Full-Time schedule, HCN nurses may have opportunities to accept:

  • RNCM / Recertifications throughout the state (+ supporting compensation)
  • Coverage for vacations and leaves
  • Supplemental nursing assignments
  • This role is best suited for individuals who desire flexibility and can accommodate a schedule that changes based on client needs.

PRN work is based upon agency and client needs and is never guaranteed. Participation is optional.

For the right candidate, this position can serve as a flexible way to maintain clinical practice while earning additional income.

*For those seeking only PRN, please note that: Hours vary significantly from week to week while some weeks may offer multiple available shifts, or limited hours, or no available hours at all. No minimum number of hours is guaranteed for PRN positions.


Typical Shift Availability

Most available assignments occur during:

  • Weekdays (Monday through Friday)
  • Morning start times typically between 7:00 AM and 8:00 AM
  • Afternoon/evening end times typically between 3:00 PM and 6:00 PM
    • Occasional late evening visits may be necessary, with advance notice.
  • Weekend opportunities may occasionally become available but are not guaranteed.
  • Overnight opportunities are rare.

Shift lengths and schedules vary by client and assignment, and may vary day-to-day, week-to-week for PRN Caregivers.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred.
  • Minimum of one (1) year Experience in case management is required.
  • Strong assessment and communication skills
  • Commitment to quality patient care
  • Possesses a strong track record of attendance and accountability, with a desire for long-term employment with our agency
  • Excellent documentation habits and Plan of Care adherence
  • CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability track record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current Driver's License and reliable transportation.
  • Willingness and Ability to perform all Essential Physical Demands:
    • Frequent standing, walking, stooping/bending, infrequent squatting/kneeling, and balancing, occasional sitting.
    • Weight handled: push/pull force required: 1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently; carry/lift:1-20 lbs. frequently, 20-30 lbs. occasionally, 30-50 lbs. infrequently.
  • Additional Specializations & Accommodations:

    • This role works closely with and reports directly to our NW OH Regional Program Administrator (local) and Director of Clinical & Administrative Operations (statewide).
    • For those pursuing career growth: a defined path for advancement to Full-Time RNCM, Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.

Job Summary

As an RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Reports to: Regional or Clinical Administrator / Director of Nursing

Responsibilities/Requirements of an RN Case Manager / Recertification Nurse include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.
