At Concho Hearts Hospice, our Social Workers make a meaningful difference by providing emotional support, guidance, and valuable resources to patients and families during their hospice journey.
We're looking for a compassionate and dependable PRN Hospice Social Worker to join our team serving Greenville, TX, and surrounding communities.

What You'll Do
- Complete psychosocial assessments for hospice patients and families
- Identify emotional, social, financial, and practical needs affecting patient care
- Provide emotional support, counseling, and guidance to patients and caregivers
- Assist families with community resources, advance care planning, and end-of-life concerns
- Support patients and families experiencing anticipatory grief, loss, and difficult transitions
- Collaborate with nurses, chaplains, physicians, and other members of the interdisciplinary team
- Participate in interdisciplinary team (IDT) meetings as needed
- Advocate for patient needs, preferences, and dignity
- Assist with crisis intervention and family support when appropriate
- Complete accurate and timely documentation in the EMR system
What We're Looking For
✔ Bachelor's or Master's degree in Social Work from an accredited program
✔ Current Texas Social Work License (LBSW, LMSW, or LCSW)
✔ Previous hospice, home health, healthcare, or social work experience preferred
✔ Hospice Social Work experience is a plus!
✔ Strong communication, assessment, and interpersonal skills
✔ Compassionate and comfortable supporting patients and families during difficult situations
✔ Ability to work independently while collaborating with an interdisciplinary team
✔ Strong organizational and documentation skills
✔ Valid driver's license, reliable transportation, and current auto insurance
✔ Ability to travel throughout Greenville and surrounding communities
