Description

Overview

The RCM Revenue Cycle Specialist, Credit Balance Review is responsible for auditing and validating accounts with ledger credit balances aged 30+ days with no future appointment scheduled, determining the root cause of each credit, and driving it to the correct resolution: correction, transfer, payer recoupment, patient or insurance refund, or unclaimed property processing.

In addition to core credit balance review, this role supports the refund and unclaimed property (UCP) lifecycle by documenting and initiating refunds, responding to payer overpayment requests, and performing the research and due diligence required before funds are escheated to the state. The Specialist works closely with the Refunds team, Mail team, and CBO Posting teams to ensure credits are resolved accurately, compliantly, and within required timeframes.

Responsibilities

Key Responsibilities

Credit Balance Review

Review and validate ledger credit balances aged 30+ days with no future appointment scheduled across assigned offices, using the CBR Application and Denticon.

Research each credit to determine its source (e.g., duplicate or misapplied payments, insurance overpayments, posting errors, adjustments, payments applied to the wrong patient, provider, or office).

Resolve credits through corrective transfers, adjustments, or reapplication of payments; coordinate with CBO Posting teams and practices when corrections require their action.

Participate in review rotations so that credit balances of all risk tiers (high, moderate, and low) are reviewed consistently within the established review cycle.

Document findings, actions taken, and outcomes on every account worked in the CBR Application, Denticon, and the applicable tracker.

Identify recurring root causes and trends and escalate to the Revenue Cycle Manager, Practice Support Services.

Refund Responsibilities

Initiate refund requests and FreshService tickets for confirmed patient and insurance overpayments, attaching complete supporting documentation (ledger, payment source, EOB/ERA, and payer correspondence).

  • Review refund request tickets in FreshService for accuracy, including correct Denticon data, payee, payee address, amount, and absence of other outstanding balances or pending activity on the account.
  • Audit and validate the credit in the patient ledger to confirm the refund amount is supported by the underlying payments, adjustments, and payer remittance and the correct office is charged.
  • Confirm receipt of all required documentation (e.g., EOB/ERA, payer correspondence, and approvals) before a refund is released for processing.
  • Flag each validated refund ticket as good to process and hand it off to the Refunds team; return incomplete or inaccurate tickets to the requestor with clear notes on what is needed.
  • Review and complete insurance refund documentation, and respond to payer overpayment and recoupment requests in coordination with the Refunds team.
  • Support payer refund and recoupment follow-up, including tracking checks issued, voided, returned, or reissued.
  • Post third-party finance fees and refunds as assigned, following established procedures.
  • Partner with the Refunds team to ensure completed refund requests are handed off cleanly and that reissues are minimized through accurate upfront review.

Unclaimed Property (UCP)

  • Review credit balances that cannot be refunded or resolved through normal outreach and identify accounts that meet unclaimed property criteria under applicable state requirements and DCA policy.
  • Review and maintain the UCP log, ensuring each item is accurately entered, categorized, and updated with current status.
  • Perform and document required due diligence, including address research, outreach letters, and contact attempts, within the timeframes required for the applicable state.
  • Research and resolve returned UCP letters and undeliverable refund checks, updating patient information and status accordingly.
  • Review UCP credit balances and letters and route items ready for reporting or remittance to the Refunds team with complete documentation.
  • Escalate state-specific questions, disputed claims of ownership, and exceptions to the Revenue Cycle Manager, Practice Support Services.
  • Utilize Denticon, DentalXChange (DXC), Zentist, the CBR Application, Ryan Tracker Pro, FreshService, and the Recon Portal as applicable to the role, following established workflows and documentation standards.

Respond to FreshService tickets and internal requests within established SLAs.

Participate in team meetings, training, and cross-training to provide coverage across credit balance, refund, and UCP workflows.

Contribute to SOPs, job aids, and process improvements for credit balance, refund, and unclaimed property workflows.

Adhere to HIPAA guidelines, payer rules, and internal compliance protocols; maintain clean, audit-ready documentation.

    Performance Metrics (KPIs)

Productivity: Work a minimum of 10 accounts per hour.

Recovery Rate: Maintain a 20% recovery rate on accounts worked.

Credit Balance Resolution Compliance: ≥ 95% of accounts cleared within required timeframes.

Aged Credit Balances > 120 Days: Reduced month over month within assigned offices.

Refund Validation Accuracy: Refund tickets flagged as good to process are accurate and fully documented, with refund reissues or returns attributable to Specialist review error held to a minimum.

Refund Ticket Review Timeliness: Refund request tickets reviewed and either flagged good to process or returned with notes within the established turnaround time.

UCP Due Diligence Timeliness: 100% of unclaimed property items researched, documented, and processed within the timeframe required by the applicable state.

FreshService SLA: Assigned tickets resolved within established SLA.
