Position Summary

Keeping Good Company is seeking a compassionate and dependable LPN or RN to conduct nursing visits for patients receiving in-home care. The Nurse Visit LPN or RN helps ensure patients receive quality care by completing nursing assessments, monitoring patient needs, and providing support to patients, families, and caregivers in the home setting.

This position is ideal for a nurse who enjoys working independently, providing personalized care, and helping medically fragile patients remain safe and comfortable in their homes.

Essential Duties and Responsibilities

• Conduct scheduled nursing visits in patients' homes.

• Complete nursing assessments and evaluate patients' overall condition and care needs.

• Monitor changes in patient health and communicate concerns to the appropriate care team.

• Review and update nursing documentation and patient records as required.

• Ensure care provided is consistent with the patient's plan of care and applicable nursing standards.

• Communicate with patients, families, caregivers, and the office team regarding patient needs.

• Provide education and support to families and caregivers when appropriate.

• Complete required visit documentation accurately and in a timely manner.

• Maintain patient confidentiality and comply with applicable healthcare regulations and company policies.

Qualifications

• Current, active LPN or RN Nursing License in the state of Missouri.

• Previous nursing experience preferred.

• Experience in home health, private duty nursing a plus

• Strong communication, organization, and documentation skills.

• Ability to work independently and manage scheduled visits.

• Reliable transportation and willingness to travel to patients' homes as needed.

• Compassionate, professional, and patient-focused approach to care.

• CPR Certified
