Heartline Staffing is recruiting a Family Nurse Practitioner for a full-time outpatient primary care opportunity in South Bend, Indiana. This role focuses on whole-person primary care, health promotion, disease prevention, chronic disease management, patient education, and appropriate referrals within a team-based clinical environment.

The FNP will have clinical support while maintaining meaningful autonomy in day-to-day patient care and serving an underserved patient population.

COMPENSATION
• $110,000–$130,000+ annual base salary
• Student loan repayment eligibility of $50,000–$100,000
• Relocation assistance available

SCHEDULE
• Monday–Friday
• 8:00 AM–5:00 PM
• No call
• No nights
• No weekends

POSITION OVERVIEW
• Provide comprehensive outpatient primary care.
• Support health promotion and disease prevention.
• Diagnose and manage chronic health conditions.
• Provide patient education and coordinate appropriate referrals.
• Deliver whole-person care within a team-based outpatient environment.
• Maintain provider autonomy with full clinical support staff.
• Meet a minimum of 2,946 outpatient visits annually.
• Utilize EHR-supported clinical workflows.

Requirements

Requirements
• Certified Family Nurse Practitioner REQUIRED
• 1–3 years of Nurse Practitioner experience REQUIRED
• Clear and active Indiana Nurse Practitioner license or ability to obtain one REQUIRED
• Prescriptive authority REQUIRED
• DEA registration REQUIRED
• Current CPR certification REQUIRED
• Ability to provide comprehensive, whole-person outpatient primary care REQUIRED

Preferred
• Spanish bilingual ability
