Through primary care and multispecialty medical practices, the physicians and other health care professionals of NYPMG, in collaboration with Columbia Doctors and Weill Cornell Physicians, provide families with high quality, patient-centered care close to home in the New York metropolitan area.
Care Management Coordinator – RN – Ambulatory - Days
Pursue this highly-transformational role, responsible for providing care management to high risk patients in the local community to improve the management of their chronic illness and to decrease unnecessary emergency and inpatient admissions. Conduct case-finding, assessments, education, counseling and other interventions to patients who are targeted through chronic disease registries. Assist patients in navigating NYP service delivery system and other services to overcome barriers to self-management and adherence. Connect patients and their families to community resources and ensure the proper continuation of quality care.
This is a full time position working five (5) 7.5-hour workdays between the hours of 8:00am-6:00pm at NewYork-Presbyterian/Weill Cornell Medical Center.

Education Qualifications (Required/Preferred)
BSN (Required)
Masters in Nursing or related field. If Masters in Nursing, BSN can be preferred (Preferred)
Licensure/Certifications Qualifications (Required/Preferred)
Current NYS RN Licensure (Required)
PRI Certification (Preferred)
Work Experience Qualifications (Required/Preferred)
5+ years of healthcare experience (Required)
2+ years of experience in case management in an inpatient, HMO, Insurance, or other outpatient setting (Preferred)
Experience with electronic medical records (Required)
Computer proficiency in Word and Excel (Required)
Excellent interpersonal skills (Required)
Ability to problem solve and prioritize (Required)
Flexibility and ability to work in fast-paced, changing environment (Required)
Bilingual English and Spanish (Preferred)
