Location: Layton Parkway

Hours: Monday-Friday


Job Summary

The Population Health Coordinator supports value-based care and quality improvement initiatives by identifying and helping resolve gaps in patient care. This position reviews population health data, electronic health records, and clinical documentation to verify completion of preventive services and other quality measures, identify missing information or outstanding care needs, and support accurate quality reporting.

Working collaboratively with physicians, clinical staff, and other departments, the Population Health Coordinator helps improve quality outcomes by researching patient records, maintaining accurate documentation, coordinating follow-up on identified care gaps, and supporting workflows designed to improve preventive and chronic care.


Essential Job Responsibilities

Value-Based Care & Quality Improvement

  • Support organizational value-based care goals and performance on quality measures, including HEDIS, CMS Stars, and other applicable quality programs.
  • Review population health reports and patient worklists to identify open care gaps and incomplete quality measures.
  • Identify patients due for preventive screenings, chronic disease monitoring, or other services associated with applicable quality measures.
  • Research patient records to determine whether recommended services have been completed and appropriately documented.
  • Locate screening results, test results, and other clinical documentation needed to accurately resolve HEDIS and preventive care gaps.
  • Coordinate follow-up on identified care gaps in accordance with established Population Health workflows.
  • Assist with preventive care initiatives, quality improvement projects, and other population health activities as assigned.

Data Management & Quality Reporting

  • Review electronic health records (EHR) and other available data sources to verify patient information, medical history, completed services, and quality-measure documentation.
  • Enter, update, or document information necessary to accurately reflect completed care and quality-measure status.
  • Identify missing, incomplete, or inconsistent information and complete appropriate research or follow-up to resolve discrepancies.
  • Maintain accurate and timely documentation within applicable EHR, population health, and reporting systems.
  • Track assigned care gaps and follow-up activities through completion or appropriate resolution.
  • Support accurate quality reporting by maintaining complete and reliable patient and quality data.

Collaboration & Professional Relationships

  • Collaborate with physicians, clinical staff, and other departments regarding identified care gaps, missing documentation, and patient follow-up needs.
  • Develop effective and dependable working relationships with providers and cross-functional clinic staff.
  • Communicate quality-measure requirements and needed follow-up clearly and professionally.
  • Support clinic workflows and initiatives designed to improve completion of preventive and chronic care services.
  • Contribute to department and organizational initiatives and perform other related duties as assigned.

Compliance & Professional Development

  • Maintain strict patient confidentiality and comply with HIPAA and organizational privacy requirements.
  • Follow established organizational policies, procedures, and quality standards.
  • Maintain knowledge of population health, value-based care, applicable quality measures, and related systems through training and continuing education.


Requirements

Performance Requirements

Knowledge

  • Knowledge of medical terminology preferred.
  • Basic understanding of preventive care, chronic disease monitoring, and healthcare quality measures preferred.
  • Understanding of value-based healthcare, Accountable Care Organizations (ACOs), HEDIS, CMS Stars, and population health concepts preferred.
  • Knowledge of continuous quality improvement principles preferred.
  • Knowledge of patient confidentiality and HIPAA requirements.

Skills

  • Strong attention to detail and accuracy when reviewing patient records and healthcare data.
  • Strong research and problem-solving skills.
  • Proficiency with Microsoft Office, including Word and Excel.
  • Experience using electronic health record systems; experience with population health management tools preferred.
  • Strong computer skills and ability to learn new software applications.
  • Effective written and verbal communication skills.
  • Strong organizational and time-management skills.

Abilities

  • Ability to prioritize and manage a high volume of patient records, care gaps, and follow-up activities.
  • Ability to identify missing or inconsistent information and determine appropriate next steps.
  • Ability to work independently while collaborating effectively with physicians, clinical staff, and other departments.
  • Ability to manage multiple assignments and deadlines with minimal supervision.
  • Ability to communicate professionally and build effective working relationships across the organization.
  • Ability to maintain confidentiality and exercise appropriate judgment when handling protected health information.

Education & Experience Requirements

Education

  • High school diploma or equivalent required.
  • Associate's or bachelor's degree in a healthcare or related field preferred.

Experience

  • One to two years of experience in a healthcare setting, medical clinic, health information, care coordination, or quality improvement environment preferred.
  • Experience using electronic health records preferred.
  • Experience with population health management tools, quality reporting, HEDIS, CMS Stars, or value-based care preferred.

Physical & Environmental Requirements

Physical Demands

  • Primarily sedentary work involving prolonged periods of sitting and computer use, with occasional standing and walking.
  • Must be able to perform the essential functions of the position with or without reasonable accommodation.

Environment

  • Professional, well-lighted medical office or administrative setting.
  • Work requires frequent use of computers and electronic health information systems.

Attendance

  • Regular and reliable attendance is an essential function of the job.

Transportation

  • Reliable transportation is required for occasional travel to satellite clinic locations for collaboration with providers and clinic staff.
