Admission Coordinator

Admission Coordinator - Non-Clinical

Admission Role would include:

1. Review all referrals submitted by hospitals for admission to rehab and/or LTC
2. Open communication with hospital discharge planners on acceptance or denial of patient referrals
3. Ensure required referral information and data are obtained timely prior to an admission arriving
4. Entering patients in the EMR
5. Follow up daily on all pending referrals
6. Completion admission paperwork
7. Bridge communication with the team regarding new admissions
8. Basic understanding of insurance, electronic medical records and medical terminology preferred
9. General use of Microsoft Office Tools

 

Qualifications:
Must have a genuine interest in geriatrics and assuring quality medical records of residents. Ability to adapt to frequent changes in routine.

Experience: Minimum of three years of related experience.  Prefer LPN, RN, MSW, but college courses would be acceptable.

Education: High school graduate or equivalent.

Physical Requirements: Must be able to lift up to 50 lbs. Ability to walk, climb stairs, push and/or pull equipment and residents as needed. Prolonged standing or walking. Able to maintain high energy level throughout an eight hour shift. This position requires the ability to perform the essential functions described.
