Your Opportunity

The Way Back Program is a non-clinical support service focused on providing rapid, practical psychosocial support to people experiencing a suicidal crisis or who have attempted suicide.
We are seeking a compassionate and dedicated Care Coordinator to join our team. In this rewarding role, you will work alongside Peer Workers and community partners to provide person-centred support that empowers individuals to achieve their goals and reconnect with their communities. As a Care Coordinator, you will work closely with people experiencing suicidal distress and their support networks, helping them access the services, resources and connections they need to support their recovery and healing. You will provide holistic, strengths-based support that promotes independence, resilience, community participation and wellbeing
Your key responsibilities will be to:
- Undertake initial intake and safety planning for clients and work with individuals and their families to create individualized case plans.
- Provide ongoing support sessions with clients and review progression and safety as required.
- Support and promote family, carers, friends and kin involvement in safety and support planning in collaboration with clients
- Liaise with other support services and organisations to coordinate the requirements of the case plan.
- Deliver group programs which support the practical needs of the clients
- Assist clients in the process of transition out of the service into independence or other services.
- Ensure documentation is completed and updated in line with Mission Australia protocols.
Requirements for Success
- Relevant tertiary qualifications and/or significant relevant work experience
- Experience working with people experiencing mental health challenges, suicidality, trauma and psychosocial support needs
- Demonstrated experience supporting individuals from diverse backgrounds, including those experiencing disadvantage and social exclusion
- Knowledge of local crisis intervention and community support services and the ability to connect people with the right supports
- Experience working with a peer workforce, with a focus on trauma-informed, recovery-oriented practice and valuing lived experience
- Excellent communication, relationship-building and case management skills.
- Computer literacy preferably using Windows.
- Current NSW Driver’s License
- Current First Aid Certificate
Before starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
