Join Mindify Wellness and Care
Mindify Wellness and Care is seeking a compassionate, outgoing, and bilingual Patient Care Coordinator to help older adults access psychiatric, therapy, and behavioral health services throughout Northern and Central New Jersey.
This role is ideal for someone who enjoys building relationships, educating others, and making a real difference in the lives of seniors and their families.
You'll work directly within our partner adult day care centers as well as in community and in-home settings, helping participants understand available services, connecting individuals to care, and serving as a trusted resource within the community.
This is a travel-based position supporting coverage needs across multiple locations, including adult day care centers and in-home visits. Mileage reimbursement is provided for travel to in home settings.
What You'll Do
- Visit partner adult day care centers throughout New Jersey
- Conduct in-home visits as needed to support patient engagement and access to care
- Build relationships with participants, families, and center staff
- Educate individuals about available behavioral health services
- Help connect patients with psychiatric and therapy services
- Support patient engagement and continuity of care
- Serve as a liaison between patients, families, providers, and center staff
- Provide Spanish-English interpretation when needed
- Maintain accurate documentation and uphold HIPAA standards
What We're Looking For
Required
- Fluent in English and Spanish
- Excellent active listening and interpersonal skills
- Ability to build relationships and adapt to different environments, policies, and procedures across adult day centers and community settings
- Comfortable speaking with individuals and groups
- Valid driver's license and reliable transportation
- Willingness to travel throughout New Jersey
- Organized, dependable, and self-motivated
