What are we looking for?
Cleveland Clinic London are looking to recruit a Case Manager to join our Case Management team with a competitive salary range on offer, dependent on experience. The role is Monday to Friday 9am – 5pm onsite.
We are seeking an experienced Case Manager to coordinate patient care and discharge planning from admission through to discharge for key patient groups. Acting as a liaison between patients, families, clinicians, and third-party payers, the postholder will support safe, timely, and cost-effective care delivery.
The role involves managing a complex patient caseload, conducting utilisation reviews and pre-authorisations, optimising length of stay and reimbursement outcomes, and working collaboratively with multidisciplinary teams to address barriers to care and discharge. The postholder will proactively identify risks to patient flow, coordinate complex transitions of care, and support achievement of clinical, operational, and financial performance objectives.

What will your duties include?
- Assess, plan, coordinate, and review patient care and discharge planning from admission through to discharge.
- Act as the key liaison between patients, families, clinicians, and third-party payers to support safe, timely, and cost-effective care.
- Manage a complex caseload, identifying and addressing barriers to discharge to achieve optimal patient outcomes, appropriate length of stay, and reimbursement.
- Develop effective working relationships with external stakeholders, including insurers, primary care providers, and external care providers, to support continuity of care.
- Contribute to service improvement initiatives, innovation, research activities, audit programmes, and the dissemination of best practice.
What we need from you?
- Bachelor's Degree in Nursing required.
- Current registration with the Nursing & Midwifery Council (First Level) with substantial post-registration experience
- Senior-level clinical experience with demonstrable leadership, service coordination, and/or team supervision responsibilities — or significant experience within the healthcare insurance sector in a clinical/case management role.
- Considerable post-registration inpatient bedside nursing experience (e.g. Oncology, acute surgical, ICU/HDU and/or neuro rehabilitation); acute secondary care experience within NHS or independent sector.
- Proven ability to lead complex clinical decision-making, work autonomously, and influence multidisciplinary teams.
