What You'll Do
As a Home Health Registered Nurse, you'll provide skilled nursing care that helps patients safely recover and manage their health in the comfort of their own homes.
You'll:
Complete comprehensive patient assessments and develop individualized care plans.
Manage a designated patient caseload while coordinating care with physicians, therapists, and other healthcare professionals.
Perform skilled nursing visits and evaluate patient progress.
Educate patients and caregivers on medications, disease management, treatments, and preventive care.
Identify changes in patient condition and adjust the plan of care as appropriate.
Document care accurately and maintain compliance with all regulatory requirements.
Advocate for patients while promoting independence, safety, and quality outcomes.
What You'll Bring
Required Qualifications
Current, unrestricted Texas Registered Nurse (RN) license
Minimum of one (1) year of nursing experience in an acute care, skilled nursing, or clinical setting
Excellent assessment, communication, and critical thinking skills
Reliable transportation, valid driver's license, and current auto insurance
Willingness to travel throughout the assigned coverage area
Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support up to 50–100 pounds, including assisting with patient positioning, transfers, and moving equipment.
Preferred Qualifications
Home Health or Hospice experience
Experience completing OASIS assessments
Homecare Homebase (HCHB) experience
Associate or Bachelor's Degree in Nursing
