Job Summary and Responsibilities
- As our LPN Case Coordinator, you will coordinate and oversee the collaborative care of patients referred to Franciscan Health System (FHS) under the direction of a physician or ARNP, working with primary care providers such as physicians, pharmacists, diagnostic imaging technicians, and physical/occupational therapists to offer guided access to the full range of services.Every day you will guide patients through intake, diagnosis, treatment, and discharge, ensure referrals and treatment plans are sent to the appropriate specialist, department, or primary care physician, coordinate resources to achieve desired clinical and financial outcomes, and work with community services and government agencies to support patients' ongoing medical needs.
- Performs patient intake and discharge activities, may include taking vital signs and turning around rooms between patients.Guides the patient through the intake, diagnosis, treatment and discharge process in collaboration with other healthcare providers in the team, the patient and family.
- Communicates with patients, physicians, and outside agencies regarding the coordination of care and treatment of patients; handles incoming phone calls, pulls charts, receives and coordinates pharmacy re-fills and handles physical/occupational therapy re-certifications.
- Addresses and manages treatment plans, referrals and pre-authorization requirements of the patient from intake through discharge to facilitate the achievement of desired clinical and financial outcomes.
- Completes documentation responsibilities in accordance with hospital and department policy..
- Performs departmental administrative functions, as needed.
- Maintains staff schedules.
- Orders supplies and equipment.

- Other Graduation from an accredited Practical Nursing program (or equivalent recognized military training) and two years of related clinical work experience that demonstrates attainment of the requisite job knowledge skills/abilities., upon hire and
- Licensed Practical Nurse: WA, upon hire and
- Basic Life Support - CPR, upon hire
