Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community. 

POSITION SUMMARY:

The Case Manager provides ongoing, documented case management to adults enrolled in Community Health & Recovery, a low-threshold program offering harm reduction, health promotion, care coordination, and recovery support for people who use drugs or are at risk of overdose or drug-related harm. Using participant-centered, trauma-informed, culturally responsive, and harm reduction approaches, the Case Manager conducts assessments, develops service plans, coordinates referrals, tracks progress, and helps participants work toward their goals. 

The role supports access to medical care, behavioral health services, substance use treatment, hepatitis care, housing, food assistance, benefits, legal services, and other health and social supports, while providing advocacy, crisis support, health promotion, and group services as appropriate. 

This role is distinct from brief drop-in harm reduction encounters because it maintains an ongoing, individualized case management relationship with each participant. 

DUTIES & RESPONSIBILITIES:

  • Engage eligible participants through respectful, nonjudgmental, participant-centered outreach and relationship-building. 
  • Maintain an assigned caseload and provide ongoing case management from enrollment through case closure. 
  • Complete assessments, determine service needs, develop and update service plans, and monitor participant progress. 
  • Coordinate referrals, warm handoffs, follow-up, advocacy, and navigation for medical, behavioral health, substance use, hepatitis/infectious disease, housing, benefits, food, legal, employment, and other support services. 
  • Provide harm reduction and health promotion education, including overdose prevention, naloxone, safer substance use, wound care, hepatitis, HIV/STI prevention, recovery, and wellness. 
  • Support access to harm reduction supplies and services, including naloxone, safer use supplies, drug checking resources, and syringe services. 
  • Maintain ongoing participant contact and provide re-engagement outreach when participants disengage. 
  • Respond to urgent participant needs using agency crisis response procedures and seek supervisory, clinical, medical, or emergency support when needed. 
  • Collaborate with CHASI programs, external providers, treatment programs, recovery supports, housing providers, and community partners. 
  • Maintain timely, accurate, and confidential documentation in agency and funder-designated systems. 
  • Assist with case conferences, supervision, training, program monitoring, audits, quality assurance, reporting, and performance improvement activities. 
  • Perform all duties in accordance with CHASI policies, funder requirements, confidentiality standards, harm reduction best practices, and applicable laws and regulations.

Requirements

  • High school diploma or equivalent with least 1 year of experience providing case management, care coordination, navigation, or closely related direct services. 
  • Experience supporting people affected by substance use, overdose risk, housing instability, food insecurity, benefits needs, or other physical, mental, social, or emotional health-related needs. 
  • Working knowledge of harm reduction, trauma-informed care, motivational interviewing, strengths-based practice, and person-centered service delivery. 
  • Ability to complete assessments, develop service plans, coordinate referrals, follow up on outcomes, and document services accurately and on time. 
  • Ability to navigate Microsoft Office, email, internet-based tools, and electronic health record or case documentation systems. 
  • Good organizational skills, attention to detail, and ability to manage multiple priorities. 
  • Effective verbal and written communication skills, including the ability to follow instructions, listen actively, organize information clearly, and communicate with participants, staff, and community partners. 

Preferred Qualifications

  • Associate or bachelor’s degree in a relevant field, or equivalent related experience. 
  • CASAC, CASAC-T, or CRPA credential. 
  • Bilingual English/Spanish skills.

The pay range for this role is?generally?$41,600-$46,600?annually,?commensurate?with relevant experience and qualifications and in alignment with internal equity.
