What You’ll Do:
- Manage a caseload of hospice patients in the College Station area.
- Provide skilled nursing care in patients’ homes and facilities.
- Complete routine visits, admissions(RN), and recertifications(RN).
- Develop and update individualized plans of care(RN).
- Collaborate with physicians and the interdisciplinary hospice team.
- Provide education and emotional support to patients and families.
- Maintain accurate documentation using a company-provided tablet.
What We’re Looking For:
- Active Texas nursing license (RN or LVN).
- Strong assessment and critical thinking skills.
- Excellent communication and teamwork.
- Ability to work independently in the field.
- Reliable transportation and willingness to travel locally.
- Most importantly, we’re looking for nurses who lead with compassion, patience, and empathy.
